生理性支抗矫治技术对拔牙病例颅颌面垂直向结构的影响 被引量:1

The effects of PASS technique on cranio maxillofacial vertical structure in tooth extraction cases

摘要 目的研究生理性支抗矫治技术对拔牙矫正治疗前后颅颌面垂直向结构的影响。方法选择2017年1月-2019年9月本院收治的减数4颗第一前磨牙的恒牙列患者80例作为研究对象,分为生理性支抗矫治技术治疗组(A组)和MBT矫治技术治疗组(B组)两组,每组各40例。对80例患者矫治前后头颅侧位片进行头影测量,对测量值进行统计分析,比较不同矫治技术对患者治疗前后颅颌面垂直向结构的影响。结果在生理性支抗矫治技术组中,U6-PP(mm)、L6-MP(mm)增大且差异具有显著性(P<0.01),L1-MP(°)减小且差异具有显著性(P<0.01);在MBT矫治技术组中,L1-MP(°)减小且差异具有显著性(P<0.01),U6-PP(mm)、L6-MP(mm)增大且差异具有显著性(P<0.01),U1-PP(mm)增加且差异有统计学意义(P<0.05);两组差异的比较中,MP-PP、U6-PP(mm)、L6-MP(mm)的差异有高度统计学意义(P<0.01),ANS-Me(mm)差异存在统计学差异(P<0.05)。结论在拔牙病例的治疗中,生理性支抗矫治技术相较于MBT矫治技术的垂直向控制能力佳,能够更好地控制磨牙和下颌平面的位置同时减少面下三分之一高度的增加。 Objective To evaluate the effects of physiologic anchorage spee’s-wire system(PASS) technique on cranio maxillofacial vertical structure after tooth extraction treatment.Methods 80 cases of patients with permanent dentition, those who received extraction of the four first premolars during January 2017 and September 2019, were enrolled in the study. According to the treatment method they were divided into PASS technique group(group A) and MBT group, 40 cases in each group. Cephalometry was performed on lateral cranial radiographs before and after correction and the measured values were statistically analyzed, and to explore the effects of different orthodontic techniques on the vertical cranial-maxillofacial structure before and after treatment.Results In the PASS technique group, U6-PP(mm) and L6-MP(mm) increased and the difference was statistically significant(P<0.01), L1-MP(°) decreased and the difference was statistically significant(P<0.01). In MBT group,L1-MP(°) decreased and the difference was statistically significant(P<0.01), U6-PP(mm) and L6-MP(mm) increased and the difference was statistically significant(P<0.01), U1-PP(mm) increased and the difference was statistically significant(P<0.05). The differences of MP-PP, U6-PP(mm) and L6-MP(mm) between the two groups were statistically significant(P<0.01), and the difference of ANS-Me(mm) was statistically significant(P<0.05).Conclusions In tooth extraction, PASS technique has better vertical control ability than MBT. PASS technique could control the position of the molar and the mandibular plane better and prevent the increase of the lower third of the mandibular plane.
