摘要
目的观察多囊卵巢综合征(PCOS)不孕患者体内抗缪勒管激素(AMH)、泌乳素的变化,研究PCOS不孕患者甲状腺功能、自身免疫及卵巢功能的改变情况。方法选取2016年6月至2018年1月初次于该院确诊的PCOS不孕患者80例作为不孕组,另选取84例健康女性为健康对照组,所有入选者年龄20~35岁。检测2组女性的AMH、泌乳素、甲状腺激素、性激素水平及自身免疫抗体阳性率。结果不孕组患者血清AMH水平为(14.47±5.60)ng/mL,泌乳素水平为(15.49±8.01)μg/L,促甲状腺激素(TSH)水平为(3.52±2.03)μIU/mL,促黄体生成素(LH)水平为(19.76±3.72)IU/L,雌二醇水平为(162.26±61.16)pmol/L,睾酮水平为(2.94±1.30)nmol/L,均显著高于健康对照组(t=11.600、2.947、7.224、18.001、3.086、6.789,P<0.05),而游离三碘甲状腺原氨酸(FT3)水平为(4.56±0.91)pmol/L,游离甲状腺素(FT4)水平为(15.16±3.05)pmol/L,三碘甲状腺原氨酸(T3)水平为(1.56±0.41)nmol/L,甲状腺素(T4)水平为(89.51±17.04)nmol/L,均显著低于健康对照组(t=14.681、11.996、10.937、12.232,P<0.05)。不孕组患者抗精子抗体(ASAb)、抗子宫内膜抗体(AEMAb)、抗卵巢抗体(AOVAb)、抗人绒毛膜促性腺激素抗体(AhCGAb)的阳性率分别为28.75%、30.00%、43.75%、21.25%,均显著高于健康对照组(χ2=5.107、9.482、20.871、8.256,P<0.05)。不孕组患者血清卵泡刺激素(FSH)水平为(6.45±2.01)IU/L,与健康对照组比较差异无统计学意义(t=1.764,P>0.05)。结论PCOS不孕患者体内AMH、泌乳素均出现不同程度的升高,甲状腺功能和卵巢功能明显减退,自身免疫抗体阳性率较高,治疗时须考虑降低患者体内的不孕抗体。
Objective To observe the changes of anti-Müllerian hormone(AMH)and prolactin in patients with polycystic ovary syndrome(PCOS)infertility,and to study the changes of thyroid function,autoimmunity and ovarian function in patients with PCOS infertility.Methods A total of 80 cases of PCOS infertility patients who were first diagnosed in the hospital from June 2016 to January 2018 were selected as infertility group,and 84 healthy women were selected as healthy control group.The candidates were all aged 20 to 35 years old.The levels of AMH,prolactin,thyroid hormone,sex hormone and autoimmune antibody positive rates were detected in the two groups of women.Results The serum AMH level of patients in the infertility group was(14.47±5.60)ng/mL,the level of prolactin was(15.49±8.01)μg/L,the level of thyroid stimulating hormone(TSH)was(3.52±2.03)μIU/mL,the level of luteinizing hormone(LH)was(19.76±3.72)IU/L,the level of estradiol was(162.26±61.16)pmol/L,and the level of testosterone was(2.94±1.30)nmol/L,which were significantly higher than those of the healthy control group(t=11.600,2.947,7.224,18.001,3.086,6.789,P<0.05),while the free triiodothyronine(FT3)was(4.56±0.91)pmol/L,and the free thyroxine(FT4)was(15.16±3.05)pmol/L,triiodothyronine(T3)was(1.56±0.41)nmol/L,and thyroxine(T4)was(89.51±17.04)nmol/L,which were significantly lower than those in the healthy control group(t=14.681,11.996,10.937,12.232,P<0.05).The positive rates of anti-sperm antibody(ASAb),anti-endometrial antibody(AEMAb),anti-ovarian antibody(AOVAb),and anti-human chorionic gonadotropin antibody(AhCGAb)were 28.75%,30.00%,43.75%and 21.25%,which were significantly higher than the healthy control group(χ2=5.107,9.482,20.871,8.256,P<0.05).The level of serum follicle stimulating hormone(FSH)in the infertile group was(6.45±2.01)IU/L,which was not significantly different from that in the healthy control group(t=1.764,P>0.05).Conclusion AMH and prolactin in patients with PCOS infertility are increased to varying degrees,thyroid function and ovarian function are significantly reduced,and the positive rate of autoimmune antibodies is high.It is necessary to consider reducing the infertility antibodies in patients.
