摘要
卵巢癌是导致女性死亡的常见妇科恶性肿瘤之一,由于其起病隐匿,且早期难以诊断,大多数卵巢癌患者诊断时已属晚期,手术及化疗效果不理想。中性粒细胞与淋巴细胞比值(neutrophil-lymphocyte ratio,NLR)是近年研究较多的炎性指标之一,研究发现术前高水平的NLR与卵巢癌患者的早期诊断、癌症分期及分级、无进展生存期、总体生存率及手术或化疗的治疗效果等密切相关,是影响卵巢癌患者预后的独立危险因素。近年来,有大量关于NLR在肿瘤的发生发展过程中的机制的相关研究。肿瘤细胞可能通过分泌IL-17募集大量中性粒细胞至肿瘤局部微环境,中性粒细胞可通过分泌基质金属蛋白酶、IL-17及TGF-β等细胞因子诱导肿瘤细胞生长和紧密性,促进肿瘤新生血管形成以及诱导上皮-间质转化的发生等。此外,中性粒细胞还可通过抑制免疫作用帮助肿瘤细胞免疫逃逸。本文就NLR在卵巢癌患者诊治与预测价值以及NLR与肿瘤发生发展过程中的机制予以综述。
Ovarian cancer is one of the common tumors with high mortality in gynecological malignant tumors.Due to its insidious onset,most ovarian cancers are diagnosed at advanced stage.The effects of surgery or chemotherapy are not satisfactory.Neutrophil-lymphocyte Ratio(NLR)is an inflammatory index that has been studied in recent years.Many studies suggest that preoperative highlevel NLR is closely related to the diagnosis,cancer staging,progression-free survival,overall survival and the therapeutic effect of surgery or chemotherapy of ovarian cancer patients.Thus NLR is an independent risk factor for the prognosis of patients with ovarian cancer.In addition,in recent years,the mechanism of NLR in the development and progression of tumor has also been studied.Tumor cells may recruit a large number of neutrophils to the local micro-environment of the tumor by secreting IL-17.Neutrophils may induce tumor cell growth and tightness,promote tumor neovascularization and induce the occurrence of EMT by secreting cytokines such as MMP,IL-17 and TGF-β.In addition,neutrophils may also help tumor cells to escape immune by inhibiting immunity.That is to say,NLR affects the occurrence and development of tumor through various ways.
