乙型肝炎病毒DNA含量对妊娠结局的影响

摘要 目的研究分析乙型肝炎病毒DNA含量对妊娠结局的影响。方法选取2017年1月~2019年1月惠州市妇幼保健计划生育服务中心产科检测出乙肝病毒(HBV)的270例产妇作为研究对象,记录270例产妇乙肝病毒的DNA含量。按照HBV-DNA含量的高低分为三组,分别为:A组(HBV-DNA>1×10~6copies/m,n=94),B组(5×10~2copies/ml≤HBV-DNA≤1×10~6copies/ml,n=86)和C组(HBV-DNA<5×10~2copies/ml,n=90)。比较三组产妇的妊娠期高血压、妊娠期糖尿病、胎膜早破、产后出血等并发症的发生情况,总结HBV-DNA含量对产妇的不良影响;比较三组新生儿早产、新生儿窒息、低体重儿、死胎等发生情况,总结HBV-DNA含量对围生儿的不良影响。结果 A组的妊娠期高血压、妊娠期糖尿病、胎膜早破、产后出血等并发症发生率高于B组和C组,B组的妊娠期高血压、妊娠期糖尿病、胎膜早破、产后出血等并发症发生率高于C组,差异均有统计学意义(P<0.05)。A组的新生儿早产、新生儿窒息、低体重儿发生率高于B组,差异有统计学意义(P<0.05);A组的新生儿早产、新生儿窒息、低体重儿发生率高于C组,差异有统计学意义(P<0.05);B、C两组的新生儿早产、新生儿窒息、低体重儿发生率比较,差异无统计学意义(P>0.05)。A、B、C三组的死胎发生率比较,差异无统计学意义(P>0.05)。结论乙型肝炎病毒的DNA含量增高,产妇的妊娠期高血压、妊娠期糖尿病、胎膜早破、产后出血等并发症发生率随之升高,新生儿早产、新生儿窒息、低体重儿等发生率也升高,对妊娠结局造成的不良影响增大。 Objective To study and analyze the effect of hepatitis B virus DNA content on pregnancy outcome. Methods From January 2017 to January 2019, 270 parturients with HBV detected in the Department of Obstetrics, Maternal and Child Health Care Family Planning Service Center of Huizhou City were selected as the research objects. The DNA contents of 270 parturients were recorded. According to the level of HBV-DNA content, they were divided into three groups: group A(HBV-DNA>1×1~06 copies/m, n=94), group B(5×10~2 copies/ml≤HBV-DNA≤1×10~6 copies/ml, n=86)and group C(HBV-DNA<5×10~2 copies/ml, n=90). The complications of pregnancy hypertension, gestational diabetes,premature rupture of membranes and postpartum hemorrhage were compared among three groups, and the adverse impact of HBV-DNA content on the parturient were summarized. The incidence rates of premature delivery, neonatal asphyxia, low birth weight and stillbirth were compared among three groups, and the adverse impact of HBV-DNA content on the perinatal was summarized. Results The incidence rates of complications such as gestational hypertension,gestational diabetes, premature rupture of membranes and postpartum hemorrhage in group A were higher than those in group B and C, and the incidence rates of complications such as gestational hypertension, gestational diabetes, premature rupture of membranes and postpartum hemorrhage in group B were higher than those in group C, the differences were statistically significant(P<0.05). The incidence rates of premature delivery, neonatal asphyxia and low birth weight in group A were higher than those in group B, the differences were statistically significant(P <0.05). The incidence rates of premature delivery, neonatal asphyxia and low birth weight in group A were higher than those in group C, the differences were statistically significant(P<0.05). There were no significant differences in the incidence rates of premature delivery, asphyxia and low birth weight between the two groups(P >0.05). There were no significant differences in the incidence rates of stillbirth among the three groups(P>0.05). Conclusion With the increase of hepatitis B virus DNA content, the incidence of complications such as pregnancy hypertension, gestational diabetes, premature rupture of membranes, postpartum hemorrhage and the incidence of premature delivery, neonatal asphyxia, low birth weight of newborn also increase, which will increase the adverse impact on pregnancy.
