摘要
目的探讨短时程脊髓电刺激治疗带状疱疹后神经痛(PHN)的效果及对患者睡眠质量的影响。方法选取2017年1月至2019年1月江苏省张家港市第一人民医院收治的94例急性PHN患者作为研究对象,完全随机分为对照组和观察组,各47例,观察组经皮硬膜外植入电极,给予短时程脊髓电刺激治疗,对照组行神经阻滞治疗,比较2组患者不同时期的疗效、睡眠质量,不同年龄段患者接受电刺激治疗的周期,并对其相关性进行观察和分析。结果观察组患者治疗后1、2、3 d及1周和1、3个月的疼痛视觉模拟量表(VAS)评分、匹兹堡睡眠质量指数(PSQI)评分均显著低于对照组[治疗后3个月:(0.8±0.7)分比(3.0±0.9)分、(5.4±1.3)分比(7.4±1.3)分],差异均有统计学意义(均P<0.05),并且2组VAS、PSQI评分随着治疗时间的增加呈现出了明显的下降趋势。短时程脊髓电刺激治疗周期随着患者年龄的增长而呈现延长趋势,35~44岁患者平均治疗周期为(11.5±0.5)d,73~82岁患者为(17.8±0.2)d。结论采取短时程脊髓电刺激治疗PHN的临床效果较好,对患者的疼痛程度可起到快速有效的缓解效果,有效改善患者的睡眠质量,安全性较高。
Objective To investigate the effect of short-term spinal cord electrical stimulation on pain and sleep quality in patients with post-herpetic neuralgia( PHN). Methods A total of 94 patients with acute PHN admitted to Zhangjiagang First People’ s Hospital from January 2017 to January 2019 were randomly divided into control group and observation group,with 47 cases in each group. The observation group was treated with short-term spinal cord electrical stimulation. The control group had nerve block therapy. Clinical efficacy and quality of sleep were analyzed between groups. Treatment duration of electrical stimulation was analyzed among patients of different ages. Results The 1,2,3 d,1-week,1-and 3-month scores of pain visual analogue scale(VAS) and Pittsburgh sleep quality index(PSQI) in the observation group were obviously lower than those in the control group[scores at 3 months after treatment:(0. 8 ± 0. 7) vs(3. 0 ± 0. 9),(5. 4 ± 1. 3) vs(7. 4 ± 1. 3)](all P < 0. 05). PSQI and VAS scores decreased during treatment. Duration of electrical stimulation therapy was(11. 5 ±0. 5)d in 35-44 years old patients and it was(17. 8 ± 0. 2) d in 73-82 years old patients. Conclusions Short-term spinal cord electrical stimulation is safe and effective in the treatment of PHN. It can quickly alleviate pain and significantly improve sleep quality.
