摘要
目的探究营养指导结合功能锻炼对骨盆骨折患者营养状况的影响。方法选取2017年1月至2019年1月空军军医大学唐都医院骨科收治的98例骨盆骨折患者作为研究对象,按照随机数表法分为对照组和观察组,每组49例,对照组给予常规功能锻炼干预,观察组给予营养指导结合功能锻炼干预。比较两组患者术后第1天、第7天、第14天营养指标[总蛋白(TP)、白蛋白(ALB)、血红蛋白(Hb)]水平以及住院期间护理满意度和并发症发生情况。结果重复测量数据的方差分析结果显示,TP、ALB、Hb时间因素及时间因素和分组的交互作用比较差异均有统计学意义(P<0.05);事后LSD-t比较显示,观察组患者术后第7天、术后第14天的TP、ALB、Hb水平分别为(63.60±5.50) g/L、(32.93±4.40) g/L、(104.38±8.55) g/L、(69.59±6.70) g/L、(44.89±4.31) g/L、(123.87±7.37) g/L,明显高于术后第1天水平[(54.29±5.10) g/L、(28.04±3.67) g/L、(104.38±8.55) g/L],对照组患者术后第7天、术后第14天的TP、ALB、Hb水平分别为(61.39±8.25) g/L、(33.71±5.20) g/L、(115.32±7.57) g/L、(66.38±9.26) g/L、(37.35±4.63) g/L、(118.51±8.36) g/L,明显高于术后第1天水平[(54.70±6.24) g/L、(27.37±3.25) g/L、(104.48±8.37) g/L],两组患者术后第14天的TP、ALB、Hb水平明显高于术后第7天,差异均有统计学意义(P<0.05);观察组患者术后14 d的TP、ALB、Hb水平明显高于对照组,差异均有统计学意义(P<0.05);观察组患者的护理总满意度为93.88%,明显高于对照度的79.59%,差异有统计学意义(P<0.05);观察组患者术后并发症发生率为10.20%,明显高于对照组的30.61%,差异有统计学意义(P<0.05)。观察组患者术后并发症发生率为10.20%,明显低于对照组的30.61%,差异有统计学意义(P<0.05)。结论对骨盆骨折患者实施营养指导干预结合功能锻炼有助于改善患者术后营养状况,提高临床护理满意度,降低术后并发症的发生率,值得临床推广。
Objective To explore the effect of nutritional guidance combined with functional exercise on the nutritional status of patients with pelvic fracture. Methods From January 2017 to January 2019, 98 patients with pelvic fracture admitted to the Department of Orthopaedics, Tangdu Hospital, Air Force Medical University were selected as the study objects. According to the random number table, they were divided into control group and observation group,with 49 patients in each group. The control group was given routine functional exercise intervention, and the observation group was given nutrition guidance combined with functional exercise intervention. The nutritional indexes(total protein [TP], albumin [ALB], hemoglobin [HB]) levels, nursing satisfaction, and complications were compared between the two groups on the 1st, 7th, and 14th day after operation. Results Variance analysis showed that TP, ALB, Hb time factors and the interactions between time factors and groupings all had statistically significant difference(P<0.05).LSD-t showed that TP, ALB, and Hb levels in the observation group were(63.60±5.50) g/L,(32.93±4.40) g/L,(104.38±8.55) g/L on the 7th day after treatment,(69.59±6.70) g/L,(44.89±4.31) g/L,(123.87 ±7.37) g/L on the 14th day after treatment, significantly higher than(54.29±5.10) g/L,(28.04±3.67) g/L,(104.38±8.55) g/L on the 1st day after surgery;the TP, ALB, and Hb levels of the control group were(61.39±8.25) g/L,(33.71±5.20) g/L, and(115.32±7.57) g/L on the 7th day after operation,(66.38±9.26) g/L,(37.35±4.63) g/L,(118.51±8.36) g/L on the 14th day after operation, which are significantly higher than(54.70±6.24) g/L,(27.37±3.25) g/L,(104.48±8.37) g/L on the 1st day after surgery;the levels of TP, ALB, and Hb on the 14th day after surgery were significantly higher in the two groups than those on the 7th day after surgery(P<0.05);the levels of TP, ALB, and Hb on the 14th day were significantly higher than those in the control group(P<0.05). The total satisfaction of nursing in the observation group was 93.88%, which was significantly higher than 79.59% in the control group(P<0.05). The incidence of postoperative complications in the observation group was 10.20% which was significantly lower than 30.61% in the control group(P<0.05). Conclusion Nutritional guidance combined with functional exercise for patients with pelvic fracture can improve the nutritional status after operation, improve the satisfaction of clinical nursing, and reduce the incidence of complications after operation, which is worthy of clinical promotion.
