摘要
目的:对先天性心脏病患儿术前的营养状况进行调查,并分析影响营养不良的因素。方法:选取2015年8月—2018年8月在绵阳市中心医院手术治疗的先天性心脏病患儿112例为研究对象,通过Z评分法对患儿营养状况进行评估,并通过相关资料和问卷调查形式对可能影响患儿营养不良的因素进行分析比较。结果:112例患儿发生营养不良共64例,营养不良发生率为57.14%,单因素分析中患儿出生时的体质量、母亲对疾病的准确感知、母亲的教育水平、焦虑程度、抑郁程度与患儿术前营养不良关系密切(P<0.05或P<0.01),Logistics回归分析显示,母亲对疾病感知度、母亲焦虑和抑郁程度以及患儿体质量是影响患儿术前营养不良的独立危险因素(P<0.01)。结论:先天性心脏病患儿术前营养不良发生率较高,其影响因素较多,根据影响营养不良的独立危险因素,对患儿的营养状况进行检测干预,同时对其母亲存在的焦虑、抑郁状况和对疾病的感知程度进行针对性干预,以改善患儿术前的营养状况。
Objective To investigate the preoperative nutritional status of children with congenital heart disease and analyze the factors affecting malnutrition.Method Totally 112 children with congenital heart disease who were treated by surgery in our hospital from August 2015 to August 2018 were selected as study objects.The nutritional status of the children was evaluated by z-score method,and the factors that might affect the malnutrition of the children were analyzed and compared through relevant data and questionnaire survey.Result Totally 112 cases of children with a total of 64 cases of malnutrition,the incidence of malnutrition was 57.14%,the single factor analysis of the body quality of child birth,mother's accurate perception of disease,the mother's education level,degree of anxiety,and depression were closely related to preoperative malnutrition children(P<0.05 or P<0.01).Logistics regression analysis showed that mother's illness perception degrees,mother children with the degree of anxiety and depression and the quality was independent risk factor for malnutrition in children with preoperative(P<0.01).Conclusion High incidence of malnutrition in children with congenital heart disease before surgery,its influence factors is more,according to the independent risk factors influencing the malnutrition,to test the children's nutrition intervention,at the same time,the mother of anxiety,depression status and degree of perception of disease targeted intervention,to improve the preoperative nutritional status of children.
