奥塔戈运动结合早期康复对脑梗死偏瘫患者FMA评分、下肢肌力及MFES评分的影响 被引量:98

Effects of Otago exercise training combined with early rehabilitation on FMA score, lower limb muscle strength and MFES score in patients with cerebral infarction hemiplegia

摘要 目的分析奥塔戈运动训练联合早期康复对脑梗死偏瘫患者简易运动功能(FMA)评分、下肢肌力及修正版跌倒效能量表(MFES)评分的影响。方法依据随机数字表法将86例脑梗死偏瘫患者分为两组,对照组(n=43)接受常规康复训练,观察组(n=43)接受奥塔戈运动训练与早期康复。进行FMA评分与Barthel指数比较,评定下肢肌力恢复情况,统计MFES评分、Berg平衡量表(BBS)评分、起立-行走计时测试(TUGT)时间、跌倒发生情况。结果干预后两组FMA评分与Barthel指数较干预前明显升高(P<0.05),且干预后观察组FMA评分与Barthel指数明显大于对照组(P<0.05);干预后两组下肢肌力0级占比均较干预前明显减少(P<0.05),下肢肌力4级占比、5级占比较干预前明显增加(P<0.05),且干预后观察组下肢肌力5级占比明显高于对照组(P<0.05);干预后两组MFES评分、BBS评分较干预前明显升高(P<0.05),TUGT时间较干预前明显减小(P<0.05),且干预后观察组MFES评分、BBS评分明显高于对照组(P<0.05),TUGT时间明显小于对照组(P<0.05);两组跌倒发生率比较,差异无统计学意义(P>0.05)。结论奥塔戈运动训练联合早期康复有利于脑梗死偏瘫患者肢体运动功能恢复与日常生活能力提高,增强下肢肌力,平衡力、移动能力均得到不同程度改善,自我跌倒效能水平明显提升,减少跌倒情况发生。 Objective To analyze the effect of Otago exercise training combined with early rehabilitation on simple fugel-meyer assessment(FMA), lower limb muscle strength and MFES score in patients with cerebral infarction hemiplegia. Methods 86 patients with cerebral infarction hemiplegia were divided into two groups according to random number table method. The control group(n=43) received routine rehabilitation training, and the study group(n=43) received Otago exercise training and early rehabilitation. FMA score and Barthel index were compared, the recovery of lower limb muscle strength was evaluated, the revised fall efficacy scale(MFES), Berg balance scale(BBS), standing-walking time test(TUGT) and the occurrence of falls were counted. Results After intervention,FMA score and Barthel index in the two groups were significantly higher than those before intervention(P<0.05), and FMA score and Barthel index in the study group were significantly higher than those in the control group(P<0.05). After intervention, the proportion of lower limb muscle strength grade 0 in the two groups were significantly lower than that before intervention(P<0.05), and the proportions of lower limb muscle strength grades 4 and 5 were significantly higher than those before intervention(P<0.05), and the proportion of lower limb muscle strength grade 5 in the study group was significantly higher than that in the control group(P<0.05). After intervention, the MFES and BBS scores of the two groups increased significantly(P<0.05), the TUGT time decreased significantly(P<0.05), and the MFES and BBS scores in the study group were significantly higher than those in the control group(P<0.05), and the TUGT time was significantly shorter than that in the control group(P<0.05). There was no significant difference in the incidence of falls between the two groups(P>0.05). Conclusion Otago exercise training combined with early rehabilitation is beneficial to the recovery of limb motor function and the improvement of daily living ability of patients with cerebral infarction hemiplegia. It can enhance lower limb muscle strength, balance and mobility ability to varying degrees. The level of self-fall efficiency is significantly improved and the occurrence of fall is reduced.
