宫腔镜下两种宫腔粘连分离术式治疗中、重度宫腔粘连疗效的meta分析 被引量:12

Meta-analysis of the efficacy of two types of TRCA in treatment of moderate to severe intrauterine adhesions

摘要 目的:比较机械性和能源性宫腔粘连分离术(TCRA)治疗中、重度宫腔粘连(IUA)患者的疗效。方法:计算机检索PubMed、EMBASE、Medline、The Cochrane Library、中国知网、万方数据库与维普7个数据库,检索从建库至2019年11月的相关文献。采用Stata 12.0统计软件进行分析。结果:最终纳入5篇文献,机械性TCRA组177例,能源性TCRA组192例。Meta分析表明:机械性TCRA组患者在改善术后3个月内月经恢复率方面优于能源性TCRA组,两种手术方式在患者术后宫腔恢复总有效率、术后6个月月经恢复率、术后2年内妊娠率方面比较,均无明显差异(P>0.05)。结论:宫腔镜下治疗中、重度IUA患者,机械性TCRA组患者在改善术后3个月内月经恢复率方面显著优于能源性TCRA组,其他方面两种手术疗效相当。 Objective:To compare the efficacy of mechanical and energy-based transcervical resection of adhesions(TCRA)in patients with moderate to severe intrauterine adhesions(IUA).Methods:Seven databases including PubMed,EMBASE,Medline,Cochrane Library,China HowNet,Wanfang and CQVIP were used to searched relevant literature from the establishment day of the database to November 2019.Stata 12.0 statistical software was used for analysis.Results:Five articled were selected for analysis,including 177 cases in the mechanical TCRA group and 192 cases in the energy TCRA group.Meta analysis showed that the mechanical gTCRA was better than the energy TCRA group in improving the menstrual recovery rate within 3 months after surgery,and there was no significant difference between the two methods in the total effective rate of postoperative uterine cavity recovery,menstrual recovery rate at 6 months after surgery,and pregnancy rate within 2 years after surgery(P>0.05).Conclusion:In the treatment of patients with moderate to severe IUA under hysteroscopy,mechanical TCRA is significantly better than energy TCRA in improving the menstrual recovery rate within 3 months after operation.In other respects the two operations were equally effective.
