慢阻肺合并肺结核患者免疫功能变化 被引量:5

Changes of immune function in patients with chronic obstructive pulmonary disease complicated with pulmonary tuberculosis

摘要 目的探讨慢阻肺合并肺结核患者免疫功能的变化。方法选择2018年1~12月在本院诊断的慢阻肺合并肺结核患者20例、单纯慢阻肺患者20例、单纯肺结核患者20例为研究对象,另选择健康体检者20例为对照组。比较各组免疫功能。结果 COPD+肺结核组IgG显著高于COPD组、对照组(P<0.05)。COPD+肺结核组IgA显著高于COPD组与对照组(P<0.05)。COPD+肺结核组与COPD组NK细胞显著低于对照组,差异有统计学意义(P<0.05)。COPD+肺结核组、COPD组及肺结核组CD4+T淋巴细胞显著低于对照组,差异有统计学意义(P<0.05)。COPD+肺结核组、COPD组及肺结核组CD8+T淋巴细胞显著高于对照组,差异有统计学意义(P<0.05),COPD+肺结核组、COPD组及肺结核组间比较,差异无统计学意义(P>0.05)。COPD+肺结核组、COPD组及肺结核组CD4+/CD8+显著低于对照组,差异有统计学意义(P<0.05),COPD+肺结核组、COPD组及肺结核组间比较,差异无统计学意义(P>0.05)。结论 COPD合并肺结核患者体液免疫反应增强,而细胞免疫受到抑制。 Objective To investigate the changes of immune function in the patients with chronic obstructive pulmonary disease complicated with pulmonary tuberculosis. Methods 20 patients with chronic obstructive pulmonary disease complicated with pulmonary tuberculosis who were diagnosed in hospital from January to December 2018, 20 patients with chronic obstructive pulmonary disease and 20 patients with pulmonary tuberculosis alone were selected as subjects. Another 20 healthy subjects were selected as the control group. The immune function was compared between each group. Results The IgG in COPD^+pulmonary tuberculosis group was significantly higher than that in COPD group and control group(P<0.05). IgA in COPD^+pulmonary tuberculosis group was significantly higher than that in COPD group and control group(P<0.05). The NK cells in the COPD^+pulmonary tuberculosis group and the COPD group were significantly lower than those in the control group, and the differences were statistically significant(P<0.05). The CD4^+T lymphocytes in the COPD^+pulmonary tuberculosis group, COPD group and pulmonary tuberculosis group were significantly lower than those in the control group, and the differences were statistically significant(P<0.05). The CD8^+T lymphocytes in COPD^+pulmonary tuberculosis group, COPD group and pulmonary tuberculosis group were significantly higher than those in the control group, and the differences were statistically significant(P<0.05). There was no statistically significant difference between COPD^+pulmonary tuberculosis group, COPD group and pulmonary tuberculosis group(P>0.05). The CD4^+/CD8^+in COPD^+pulmonary tuberculosis group, COPD group and pulmonary tuberculosis group was significantly lower than that in the control group, and the difference was statistically significant(P<0.05);there was no statistically significant difference between COPD^+tuberculosis group,COPD group and pulmonary tuberculosis group(P>0.05). Conclusion The humoral immune response of the patients with COPD complicated with pulmonary tuberculosis is enhanced,while the cellular immunity is inhibited.
