摘要
目的探讨临床护理路径(CNP)在先天性巨结肠患儿术后护理中的应用效果。方法选择茂名市人民医院小儿外科2017年3月至2018年3月期间收治的先天性巨结肠患儿60例进行前瞻性研究,上述患儿以随机数字表法分为观察组与对照组,每组30例,术后观察组给予CNP护理,对照组给予常规护理,比较两组患儿的疼痛水平、并发症发生率、满意度和住院时间。结果观察组患儿并发症发生率为3.3%,明显低于对照组的30.0%,差异均有统计学意义(P<0.05);观察组患儿的住院时间为(15.1±2.1)d,明显短于对照组的(30.1±3.2)d,差异有统计学意义(P<0.05);观察组患儿疼痛0级占70.0%,明显高于对照组的26.7%,疼痛Ⅲ级占3.3%,明显低于对照组的36.7%,差异均有统计学意义(P<0.05);观察组患儿家长对护理工作的满意度为96.7%,明显高于对照组的53.3%,差异有统计学意义(P<0.05)。结论CNP在先天性巨结肠患儿术后护理中的应用可有效降低并发症发病率,缩短住院时间,且可减轻患儿的疼痛程度,患儿家属满意度高。
Objective To discuss the clinical effects of clinical nursing pathway(CNP)in post-operative nursing of children with congenital Hirschsprung's disease.Methods A prospective study were performed in 60 cases of congenital Hirschsprung's disease who needed surgical treatment from Department of Pediatric Surgery,Maoming People's Hospital during March 2017 and March 2018.The children with congenital Hirschsprung's disease were randomly divided into the observation group(n=30)and the control group(n=30)according to random number table method,the observation group were given CNP,and the control group were given routine nursing care.The degree of pain,incidence of complications,satisfaction,length of hospital stay were contrasted between the two groups.Results The incidence of complications in the observation group was 3.3%,which was significantly lower than 30.0%in the control group(P<0.05);length of hospital stay in the observation group was(15.1±2.1)days,which was significantly shorter than(30.1±3.2)days in the control group(P<0.05);in the observation group,Grade 0 pain accounted for 70.0%,which was significantly higher than 26.7%of the control group,while theⅢgrade of pain accounted for 3.3%,which was significantly lower than 36.7%of the control group(all P<0.05);the nursing satisfaction of parents of children in the observation group was 96.7%,which was significantly higher than 53.3%in the control group(P<0.05).Conclusion CNP plays an active role in the post-operative nursing of children with congenital Hirschsprung's disease.It can effectively reduce the incidence of complications,shorten the hospitalization time,significantly alleviate the degree of pain of children with Hirschsprung's disease,and the satisfaction of children and their families is high.
