B超引导下星状神经节阻滞辅助治疗脑卒中后肩-手综合征的临床疗效 被引量:9

Clinical efficacy of B-ultrasound-guided stellate ganglion block for adjuvantly treating shoulder-hand syndrome after stroke

摘要 目的观察B超引导下星状神经节阻滞辅助治疗脑卒中后肩-手综合征的临床疗效。方法将56例脑卒中后肩-手综合征患者,随机分为对照组与观察组,每组28例。对照组给予常规综合治疗,观察组在对照组基础上进行B超引导下星状神经节阻滞。采用自我妨碍量表(SHS)、疼痛视觉模拟量表(VAS)、简化Fugl-Meyer评定量表(FMA)分别评估两组患者治疗前后水肿程度、疼痛程度和上肢运动功能;比较两组患者治疗总有效率。结果治疗前,两组患者SHS评分、VAS评分及FMA评分比较,差异均无统计学意义(均P>0.05)。治疗14 d后,观察组SHS评分、VAS评分均低于治疗前和对照组,FMA评分高于治疗前和对照组(均P<0.05),且观察组治疗总有效率高于对照组(P<0.05)。结论B超引导下星状神经节阻滞辅助治疗脑卒中后肩-手综合征疗效确切,能够显著地改善患者患侧上肢水肿、疼痛及上肢运动功能。 Objective To observe the clinical efficacy of B-ultrasound-guided stellate ganglion block for adjuvantly treating post-stroke shoulder-hand syndrome.Methods Fifty-six patients with shoulder-hand syndrome after stroke were enrolled and randomly divided into control group and observation group,with 28 cases in each group.The control group received conventional comprehensive treatment,while the observation group received B-ultrasoundguided stellate ganglion block on the basis of the therapy performed in the control group.The Self-Handicapping Scale(SHS),the Visual Analogue Scale(VAS)and the Simplified Fugl-Meyer Assessment(FMA)were separately used to evaluate edema degree,pain degree and upper extremity motor function of patients in the two groups before and after treatment;the total therapeutic effective rates of the two groups were compared.Results There were no statistically significant differences in SHS score,VAS score or FMA score between the two groups before treatment(all P>0.05).Fourteen days after treatment,the two groups obtained lower SHS score and VAS score and higher FMA score in comparison with pre-treatment and the control group(all P<0.05);besides,the total therapeutic effective rate for treatment in the observation group was higher than that in the control group(P<0.05).Conclusion B-ultrasound-guided stellate ganglion block is effective in the adjuvant treatment of post-stroke shoulderhand syndrome,can significantly improve the edema and pain of affected upper extremities,and upper extremities motor function.
