摘要
目的探讨皖北地区人群中不规则抗体阳性率及其特异性分布。方法选取拟进行输血的3039例患者血液标本进行不规则抗体筛查,并对抗体筛查阳性的标本进行抗体特异性鉴定与分析。结果3039例患者血液标本中,不规则抗体筛查阳性32例,阳性率1.05%;3039例患者中男性1648例,阳性8例,阳性率0.49%;女性1391例,阳性24例,阳性率1.72%。女性患者不规则抗体筛查阳性率显著高于男性(P<0.01)。32例不规则抗体中,20例属Rh-hr血型系统,5例属MNSs血型系统,2例属Duffy血型系统,1例属Diego血型系统,4例属Lewis血型系统;在不规则抗体特异性构成中,Rh-hr血型系统抗体数显著高于其他血型系统(P<0.01)。20例Rh-hr血型系统中2例抗-D抗体,12例抗-E抗体,5例抗-c抗体,1例抗-e抗体;在Rh-hr血型系统不规则抗体的特异性构成中,抗-E抗体数显著高于其他抗体(P<0.01)。结论皖北地区不规则抗体主要出现于女性中,且以抗-E抗体为主。在皖北地区对献血者进行E抗原检测,对患者(尤其女性)进行抗体筛查和特异性鉴定有重要临床意义。
Objective To investigate the positive rate of irregular antibodies and distribution of antibody specificity in northern Anhui Province.Methods The blood samples of 3039 patients who planned to receive blood transfusion were selected for irregular antibody screening,and the samples with positive antibody screening results were identified and analyzed for antibody specificity.Results In all the blood samples,32 cases were positive for irregular antibody screening,and the positive rate was 1.05%.Among 1648 male cases,8 cases were positive,and its positive rate was 0.49%.Among 1391 female cases,24 cases were positive,and its positive rate was 1.72%.The positive rate of irregular antibodies in female patients was significantly higher than that in male patients(P<0.01).Among 32 cases of irregular antibodies,20 cases belonged to Rh-hr blood group system,5 cases belonged to MNSs blood group system,2 cases belonged to Duffy blood group system,1 case belonged to Diego blood group system,and 4 cases belonged to Lewis blood group system.The number of antibodies from Rh-hr blood group system was significantly higher than that from other blood group systems(P<0.01).Among 20 cases of irregular antibodies from the Rh-hr blood group system,there were 2 cases of anti-D antibody,12 cases of anti-E antibody,5 cases of anti-c antibody,and 1 case of anti-e antibody.The number of anti-E antibodies was significantly higher than that of other antibodies in Rh-hr system(P<0.01).Conclusion Irregular antibodies mainly appear in women in northern Anhui Province,and are mainly anti-E antibodies.Therefore,it is of great clinical significance to detect E antigen in blood donors,screen irregular antibodies for patients(especially for women)and identify the specificity of antibodies in northern Anhui Province.
