摘要
目的探讨藿胆片联合克拉霉素对慢性鼻窦炎胆经郁热证的疗效安全性。方法选取2017年11月~2018年11月本院收治慢性鼻窦炎患者共200例,按随机数字法分成对照组和观察组各100例。两组给予克拉霉素颗粒,250 mg/次,2次/d;对照组给予鼻腔冲洗。观察组给予藿胆片,口服,3~5片/次,2~3次/d。两组连续治疗8周。比较两组鼻部主观病情视觉模拟量表(VAS)、Lund-Mackay、Lund-Kennedy、胆经郁热证症状、鼻腔鼻窦结局测试-20(SNOT-20)量表评分、临床疗效、不良反应发生。结果治疗8周后,观察组患者的VAS、Lund-Mackay、Lund-Kennedy、胆经郁热证症状(鼻塞、头痛、鼻粘膜充血、口苦咽干、鼻涕)、SNOT-20量表(鼻部症状、相关症状、睡眠障碍、情感结局)评分明显少于对照组(P<0.01);观察组的总有效率为93.94%,显著高于对照组(85.71%)(P<0.05);两组患者未见明显不良反应发生。结论藿胆片联合克拉霉素治疗慢性鼻窦炎胆经郁热证,可促进症状体征和中医证候好转,提高疗效,改善患者的生活质量,安全性好。
Objective To investigate the efficacy and safety of Huodan tablet combined with clarithromycin in treating chronic sinusitis with syndrome of stagnated heat of gallbladder channel. Method Two hundred chronic sinusitis were selected from November of 2017 to November of 2018 in hospital and randomly divided into the control group and the observation group with 100 cases of each group by random number table. Both groups orally received clarithromycin(250 mg/time and bid). Cases in the control group were given with nasal irrigation. The observation group was orally given with Huodan tablet(3~5 tablets/time and 2~3 times/d). Two groups were treated for 8 weeks. Scores of nasal subjective illness visual analogue scale(VAS), Lund-Mac Kay, Lund-Kennedy, symptoms of syndrome stagnated heat of gallbladder channel, sinus outcome test-20(SNOT-20), the efficacy and adverse reactions were compared for two groups. Results After 8 weeks’ treatment, scores of VAS, Lund-Mackay, Lund-Kennedy, symptoms(nasal congestion, headache,nasal mucosa congestion, dry throat, nasal discharge) of syndrome stagnated heat of gallbladder channel, and SNOT-20(nasal symptoms,related symptoms, sleep disorders, emotional outcomes) in the observation group were obviously lower than control group(P<0.01).The total efficacy of the observation group was 93.94%,which was obviously higher than control group(85.71%).No significant adverse reactions were observed in the two groups. Conclusion Huodan tablet combined with clarithromycin in treating chronic sinusitis with syndrome stagnated heat of gallbladder channel can promote the improvement of symptoms and signs, as well as the symptoms of traditional Chinese medicine. It can improve the efficacy and the quality of life of patients, and is also safe.
