摘要
目的:探究改良Hamra睑袋整复术矫治下睑袋松垂伴泪槽沟畸形的效果,及对泪槽沟结构重建的影响。方法:回顾性选取笔者医院收治的行改良Hamra睑袋整复术的下睑袋松垂伴泪槽沟畸形患者80例。根据Hirmand法对泪槽畸形程度进行分型,分为Ⅰ型34例,Ⅱ型46例。通过泪槽沟畸形等级量表(Tear trough rating scale,TTRS)及满意度调查评价泪槽结构的恢复及手术效果。结果:与术前相比,术后1、3、6个月TTRS评分明显下降,差异有统计学意义(P<0.05);术后6个月,两型之间比较差异无统计学意义(P>0.05)。Ⅰ型、Ⅱ型患者满意度分别为100.00%、86.96%,两型之间比较差异无统计学意义(P>0.05)。Ⅰ型患者术后修复效果良好。Ⅱ型患者中6例效果欠佳;5例下眼睑袋修复效果满意,但是泪槽畸形改善不明显,恢复为Ⅰ型状态,术后效果较满意;3例下眼睑及泪槽畸形全部复发,给予改良Hamra术,效果满意;其余32例术后修复效果满意。结论:改良Hamra睑袋整复术在下睑袋伴泪槽沟畸形患者治疗中取得较好的临床疗效,且促进患者泪槽解剖结构畸形的恢复,值得临床推广应用。
Objective To explore effect of modified Hamra blepharoplasty on the anatomical structure of lacrimal groove in patients with lower eyelid bag with lacrimal groove deformity.Methods Retrospectively selected 80 cases of lower eyelid bag with lacrimal groove deformity treated by modified Hamra blepharoplasty in our hospital.According to the degree of lacrimal groove deformity by Hirmand method,34 cases were classified as type Ⅰ and 46 cases as type Ⅱ.The recovery of lacrimal groove structure and the effect of operation were evaluated by def ormity rating scale(TTRS)and satisfaction survey.Results Compared with the preoperative,the TTRS scores of 1,3,6 months after operation were significantly lower,the differences were statistically significant(P<0.05).There was no significant difference between the two types at 6 months after operation(P>0.05).The satisfaction of type Ⅰ and type Ⅱ patients was 100.00% and 86.96%,respectively,there was no significant difference between the two types(P>0.05).The results of type Ⅰ patients were good.Among the type Ⅱ patients,6 cases had poor effect.5 cases had satisfactory effect of lower eyelid repair,but the improvement of lacrimal groove deformity was not obvious,which was restored to the type Ⅰ state,and the postoperative effect was more satisfactory.3 cases had recurrence of lower eyelid and lacrimal groove deformity,which was treated with improved Hamra operation,with satisfactory effect.The rest 32 cases had satisfactory effect of postoperative repair.Conclusion Modified Hamra blepharoplasty has a good clinical effect in the treatment of lower eyelid bag with lacrimal groove deformity,and promotes the recovery of lacrimal groove anatomical deformity,which is worthy of clinical application.
