摘要
目的:采用回顾性的横断面研究,应用免疫组织化学法检测血管内皮生长因子(Vascular Endothelial Growth Factor,VEGF)和CD34在正常舌黏膜经非典型性增生到舌鳞状上皮细胞癌(简称:舌鳞癌)的表达情况。探讨VEGF和微血管密度(microvessel density,MVD)在舌鳞癌发展过程中的作用。方法:应用免疫组织化学法检测VEGF和MVD在正常舌黏膜、舌白斑、舌鳞癌表达情况,应用SPSS l6.0统计软件进行数据分析。结果:VEGF在正常黏膜组织、非典型性增生、舌癌中的表达阳性率依次升高,组间差异有统计学意义(P<0.01)。舌鳞癌TNM分期Ⅰ期、Ⅱ期与Ⅲ期阳性率分别为36.36%、64.29%、90%,组间差异有统计学意义(P<0.05)。在有淋巴结转移组与无淋巴结转移组的舌鳞癌阳性表达率中,组间差异有统计学意义(P<0.05)。VEGF在5年内有复发的患者阳性率高于无复发患者组,差异有统计学意义(P<0.05)。VEGF表达与MVD呈正相关。结论:VEGF可能与舌鳞癌的发生发展有关,其阳性表达者预后不良。
Objective The purpose of this study aimed at investigating the expression of VEGF and CD34 as well as their relationship with tongue leukoplakia and tongue carcinoma with immunohistochemical method in order to evaluate their roles in carcinogenesis and progression of tongue carcinoma.Method Sp immunochemisty staining method was applied to detect the expression of VEGF and CD34 in normal normal tongue tissue group and tongue leukoplakia group and longue carcinoma group.The statistics and analysis of all data are treated with SPSS16.0.Results The positive expression rate of VEGF in normal tongue tissue,tongue leukoplakia and tongue carcinoma was 5.00%,30.00%,62.86%respectively.The difference among the three groups was significant(P<0.01).There was a significantly much higher positive rate of VEGF in tongue carcionma with lymph node metastasis than that tongue carcionma without lymph node metastasis(P<0.05).Furthermore our data shows that the expressions of VEGF were significantly increased in tongue carcinoma of stageⅢcomparing with tongue carcinoma of stageⅠand stageⅡ(P<0.01).The high expression of VEGF was related with the recurrence in five years.and the TNM classification(P<0.05).There was a significantly difference microvessel density count between the group of positive and negative expression of TGF-β1(P<0.05).Conclusion The expressive levels of VEGF might importantly affect on their carcinogenesis,progression and biologic behaviors of tongue cancer.Over-expression of VEGF might have poorly prognosis.
