摘要
目的探讨9条目患者健康问卷抑郁量表(PHQ-9)在产后抑郁筛查中的应用价值。方法从2017年6月~2018年6月在我院住院分娩、并在产后42 d按医嘱回访的产妇中,随机选取362例作为研究对象,分别采用PHQ-9和爱丁堡产后抑郁量表(EPDs)评定其产后抑郁状况,以美国精神障碍诊断与统计手册(4版)轴I障碍定式临床检查患者版(SCID-I/P)的抑郁障碍诊断标准作为产后抑郁诊断标准。结果将两种量表的临界值分别界定为15分和13分时,PHQ-9对产后抑郁诊断的敏感度为84.88%,特异度为81.15%,阳性预测值为58.40%,阴性预测值为94.51%;EPDS对产后抑郁诊断的敏感度为80.23%,特异度为81.88%,阳性预测值为59.00%,阴性预测值为93.00%;PHQ-9对产后抑郁诊断的准确率为81.49%,EPDS对产后抑郁诊断的准确率为82.04%。两种量表特异度、敏感度、阴性预测值、阳性预测值及准确率比较,差异均无统计学意义(P>0.05)。结论与EPDS比较,PHQ适合于产后抑郁的筛查,可作为产后抑郁筛查的自评量表。
Objective To explore the application value of the 9-question depression scale from the patient health questionnaire(PHQ-9)in the screening of postpartum depression(PPD).Methods A total of 362 women within 42 days postpartum,who were delivered in our hospital from June 2017 to June 2018,were enrolled in this study.PHQ-9 and Edinburgh postpartum depression scale(EPDS)were used to assess their postpartum depression respectively.And the structured clinical interview for DSM-IV-TR axisⅠdisorders-patient edition(SCID-I/P)was regarded the standard for postpartum depression diagnosis.Results The cut-off score for PHQ-9 and EPDS was 15 and 13,respectively.Sensitivity of PHQ-9 was 84.88%,and specificity was 81.15%,with positive pretictive value of 58.40%,negative pretictive value of 94.51%.Sensitivity of EPDS was 80.23%,and specificity was 81.88%,with positive pretictive value of 59.00%,negative pretictive value of 93.00%.Accuracy of PHQ-9 was 81.49%,accuracy of EPDS was 82.04%.The Sensitivity,specificity,positive pretictive value,negative pretictive value and accuracy showed no statistical significant differece between PHQ-9 and EPDS(P>0.05).Conclusion Compared with EPDS,PHQ-9 is suitable for postpartum depression screening and can be used as a self-rating scale for postpartum depression screening.
