摘要
扁桃体恶性肿瘤是头颈部最常见肿瘤之一,西医临床治疗以放疗为主,化疗、手术为辅,但生存率并不理想,影响预后因素多。与西医治疗相比,中医药注重整体思想,辨证施治,充分调动人体自身免疫机能发挥多靶点综合治疗作用,外治法与内治法相结合,调节机体五脏六腑、平衡阴阳、疏通气血经脉,"补其不足,泻其有余",达到"阴平阳秘"之效,在改善扁桃体癌患者临床症状方面取得了较好的疗效。目前关于中西医治疗扁桃体恶性肿瘤研究中存在诸多问题:①相关的临床研究以个案报道为主,样本量较小,缺乏大样本的临床实验研究,个别研究并未设置对照组,缺乏令人信服的循证医学资料;②中医对扁桃体恶性肿瘤的辨证分型尚未统一,疗效判定缺乏统一标准;③关于中医药治疗扁桃体癌的作用机制研究尚未见报道;④中医注重辨证论治,不同患者遣方用药存在差异性,如何实现中医药标准化还需要进一步探索。因此,中医药治疗扁桃体恶性肿瘤应进一步加大实验研究的样本量,统一疗效判定标准,深入探索其作用机制,以切中病机,准确辨证,恰当用药,控制肿瘤的生长,提高扁桃体恶性肿瘤患者生存质量,是今后研究的方向。
Tonsil malignant tumor is one of the most common tumors of the head and neck. Western medicine is mainly treated by radiotherapy,chemotherapy and surgery. But the survival rate is not ideal,and there are many factors influencing the prognosis.Compared with western medicine,Chinese medicine pays attention to the holism and the treatment based on syndrome differentiation,fully mobilizing the body’s self immune function to play the role of multi-target comprehensive treatment,combining external treatment with internal treatment,regulating the body’s five zang viscera and six fu viscera,balancing yin and yang,dredging qi,blood and channels, " reinforcing the deficiency and reducing the excess" to achieve the effect of " relative equilibrium of yinyang". It has a good effect in improving the clinical symptoms of tonsil malignant tumor patients. At present,there are many problems in the research on the treatment of tonsil malignant tumor by traditional Chinese and western medicine:(1) the related clinical studies are mainly case reports,with small sample size,lack of large sample clinical experimental studies,lack of control group in individual studies,and lack of convincing evidence-based medical data;(2) the syndrome differentiation and classification of tonsil malignant tumor in traditional Chinese medicine has not been unified,and there is no unified standard to judge the curative effect;(3) the mechanism of the treatment of tonsil malignant tumor by traditional Chinese medicine has not been reported;(4) Chinese medicine pays attention to the treatment based on syndrome differentiation,and there are differences in different patients’ prescription and medication. How to realize the standardization of traditional Chinese medicine needs further exploration. Therefore,in the treatment of tonsil malignant tumor,we should further increase the sample size of experimental study,unify the criteria of curative effect,explore the mechanism of action in order to accurately grasp the pathogenesis,accurately differentiate the syndromes,properly use medicine,control the growth of tumors,and improve the quality of life of patients with tonsil malignant tumor. It is the direction of future research.
