摘要
临床思维的不确定性是由医学的不确定性决定的,医学的不确定性来源于客观世界、社会世界和知识的不确定性,反映在临床活动的各个方面。医学的本质目的是救人,基于医学的目的和临床思维的不确定性,将“首先除外威胁生命的病症”作为临床思维的第一原则有其必要性和科学性,也是应对临床思维不确定性的首要方法;批判性思维对科学理论的解释和预测持审慎态度,是探索不确定性现实问题的最佳实践;在临床思维有限的确定性范围内,流程化思维有助于建立一个“做对事容易、做错事难”的工作流程和安全理念。
The uncertainty of clinical thinking is determined by uncertainty of medicine,which originates from the objective world,the social world and the uncertainty of knowledge,and can be seen all over clinical activities.The essential purpose of medicine is saving life.Therefore,it is necessary and scientific to take"priority of excluding life-threatening symptoms"as the first principle of clinical thinking on the basis of the purpose of medicine and the uncertainty of clinical thinking,which is also the primary method to deal with the uncertainty of clinical thinking.Critical thinking is cautious about the interpretation and prediction of scientific theory,which is the best practice to explore the uncertainty.In the limited certainty of clinical thinking,procedural thinking is conducive to building a safer health system that is"easy to do right and difficult to do wrong".
