摘要
目的探讨二甲双胍对进展期胃癌合并2型糖尿病患者行胃癌根治术后生存期的影响。方法选取2011年1月至2015年10月胃肠外科收治的进展期胃癌且合并2型糖尿病患者87例,其中42例应用二甲双胍控制血糖(试验组),45例应用其他降糖药物控制血糖(对照组),术后进行随访观察,比较2组的无病生存率(PFS)及总生存率(OS)。结果二甲双胍组进展期胃癌患者3年-DFS和OS分别为52.4%和69.0%,显著高于对照组进展期胃癌患者的31.1%和46.7%(P<0.05)。多因素COX分析显示二甲双胍是进展期胃癌合并2型糖尿病术后患者3年-DFS和3年-OS的独立保护因素(P<0.05)。结论二甲双胍可显著提高进展期胃癌合并2型糖尿病术后患者的无病生存率及总生存率,延长患者的生存时间。
Objective To investigate the effects of metformin on survival duration after radical gastrectomy in patients with advanced gastric cancer complicated by type 2 diabetes mellitus.Methods The clinical data about 87 patients with advanced gastric cancer complicated by type 2 diabetes mellitus who were diagnosed and treated in our hospital from January 2011 to October 2015 were retrospectively analyzed,who were divided into metformin group(n=42) and control group(n=45) according to the diabetes medication.The patients in metformin group were treated by metformin,however,the patients in control group were treated by the other hypoglycemic agents,with followe up for both groups.The disease-free survival(DFS) and overall survival(OS) were observed and compared between the two groups.Results The 3-year DFS and 3-year OS in metformin group were 52.4% and 69.0%,respectively, which were significantly higher than those(31.1% and 46.7%) in control group(P<0.05). Cox regression multivariate analysis showed that metformin was an independent protective factor of the 3-year DFS and 3-year OS of patients with advanced gastric cancer complicated by type 2 diabetes mellitus(P<0.05).Conclusion The metformin can improve the survival duration of patients with advanced gastric cancer complicated by type 2 diabetes mellitus,which can prolong the 3-year DFS and 3-year OS of patients.
