双低技术在iCT全脑灌注成像中的应用研究 被引量:1

摘要 目的探讨80kV低管电压及低管电流在iCT全脑灌注成像(CTP)中的可行性。方法60例全脑CTP检查者,管电压均为80kV,按照不同管电流分为3组,A组(50mAs),B组(100mAs),C组(150mAs),每组20例。记录各组的辐射剂量,比较分析每例颞叶白质、尾状核头的灌注参数〔脑血容量(CBV)、脑血流量(CBF)、平均通过时间(MTT)及达峰时间(TTP)〕、图像客观指标〔CT值、噪声(SD)、信噪比(SNR)、对比噪声比(CNR)〕及主观评分。结果A^C组的有效剂量(ED)分别为1.10mSv、2.21mSv、3.31mSv;3组颞叶白质的CBV、MTT存在统计学差异(P<0.05),颞叶白质的CBF、TTP及尾状核头的各灌注参数均无显著差异(P>0.05);3组图像质量客观指标、主观评分比较均有统计学差异(P<0.05);B组与C组的灌注参数及图像质量均无显著差异(P>0.05)。结论80kV低管电压联合100mAs低管电流行全脑CTP扫描可行性较好,不仅对灌注参数和图像质量没有显著影响,而且还显著降低了辐射剂量。 Objective To investigate the feasibility of 80 kV and low tube current on whole brain iCT perfusion imaging(CTP).Methods Sixty patients underwent 80 kV whole brain CTP examination and were divided into 3 groups according to different tube currents,group A(50 mAs),group B(100 mAs),and group C(150 mAs).The parameters of the 20 patients of each group were analyzed in terms of temporal white matter and caudate nucleus,including cerebral blood volume(CBV),cerebral blood flow(CBF),mean transit time(MTT)and time to peak(TTP).Subjective image quality,CT value,standard deviation(SD),signal to noise ratio(SNR),contrast signal to noise ratio(CNR)and radiation dose were measured and calculated in the 3 groups respectively.Results The effective dose(ED)of group A^C was 1.10 mSv,2.21 mSv and 3.31 mSv.Differences of CBV and MTT in the temporal white matter were witnessed among the 3 groups(P<0.05).There were no significant differences of the perfusion parameters for the CBF or TTP for the temporal white matter and the caudate nucleus among the 3 groups(P>0.05).There was significant difference in SD,SNR,CNR and the subjective image quality(P<0.05).There were no significant differences in perfusion parameters and image quality between group B and group C(P>0.05).Conclusion 80 kV/100 mAs whole-brain CTP would generate accurate perfusion parameter and satisfied image quality,and the radiation dose can be reduced profoundly.
