摘要
目的分析采用无托槽隐形矫治器和直丝弓矫治器的正畸患者在治疗过程中疼痛的变化规律,并比较佩戴两种矫治后患者疼痛程度的差异,指导两种矫治器的临床应用。方法选择正畸病例102例,采用随机信封法将研究对象分为A、B两组各51例,A组采用无托槽隐形矫治器,B组采用直丝弓矫治器,通过视觉模拟评分量表分别记录两组患者在佩戴矫治器后6 h及1、2、3、4、5、6、7 d的疼痛程度,通过睡眠等级评分表评价两种矫治器对睡眠的影响程度,并进行比较分析。结果A组矫治器佩戴后初始疼痛发生率低于B组,差异有统计学意义(P<0.05)。A组6 h及1、2、3、4、5、6 d疼痛程度均小于B组,差异有统计学意义(P<0.05)。两组的睡眠等级评分差异无统计学意义(P>0.05)。A组口腔溃疡发生率低于B组,差异有统计学意义(P<0.05)。A组疼痛强度在2 d达到高峰,5 d基本消失,B组疼痛强度在3 d达到高峰,随后逐渐降低,7 d基本消失。结论在佩戴两种矫治器后疼痛具有慢慢增加,然后逐渐降低的规律性,这种疼痛对睡眠不会造成影响。佩戴无托槽隐形矫治器产生的疼痛较轻,持续时间短,不容易引起口腔溃疡。
Objective To analyze the change rule of pain during orthodontic treatment using clear aligner and straight wire appliance,and compare the differences in pain degree between the two types of appliances after treatment,thus guiding clinical application of the two appliances.Methods One hundred and two orthodontic cases were chosen for the study and were randomly divided into group A and group B,51 in each group.The group A adopted clear aligner and the group B adopted straight wire appliance.Visual analogue scale(VAS)was used to record the pain degree of the two groups after 6 hours,1 to 7 days of wearing the appliances.A sleep rating scale was used to assess the influence of the two types of appliances on sleep,and comparative analysis was performed.Results The incidence of initial pain after wearing the appliances in the group A was lower than that in the group B(P<0.05).Pain degrees in the group A after 6 hours,1 to 7 days were all less than those in the group B(P<0.05).There was no statistically significant difference in sleep rating scale between the two groups(P>0.05).The incidence of oral ulcers in the group A was lower than that in the group B(P<0.05).The pain intensity of the group A peaked on the 2^nd day and disappeared on the 5^th day while the pain intensity of the group B peaked on the 3^rd day,then gradually decreased,and basically disappeared on the 7^th day.Conclusion The pain is gradually increased after wearing the two types of appliances,and then gradually decreased,which shows certain regularity.Moreover,the degree of pain caused by the two appliances does not affect the sleep of patients.Comparing the two appliances,the clear aligner has a lower pain,a shorter duration of pain,and is less likely to cause oral ulcers.
