摘要
目的观察中药配方颗粒制备通便膏方治疗慢性功能性便秘的疗效及对生活质量的影响。方法将120例慢性功能性便秘患者随机分为对照组(60例)和观察组(60例)。观察组给予中药配方颗粒制备通便膏方治疗,对照组给予四磨汤治疗,2组疗程均为4周。观察2组治疗前后便秘症状积分、Wexner便秘评分、心理状态、Bristol大便性状分型、生活质量的变化,并比较2组临床疗效、治疗依从性和成本-效果比。结果2组治疗后自主排便、便意感、排便费力程度、大便性状、排便不尽感、肛门阻塞感、排气不畅感、自行排便频率、每次排便时间、肛门坠胀感、腹胀评分和总分均较治疗前显著降低(P均<0.05),观察组前7项评分和总分均显著低于对照组(P均<0.05)。2组治疗后Wexner便秘评分、焦虑自评量表(SAS)和抑郁自评量表(SDS)评分较较治疗前显著降低(P均<0.05),观察组治疗后以上评分均显著低于对照组(P均<0.05);2组治疗后Bristol大便性状分型均较治疗前显著改善(P均<0.05),观察组改善情况显著优于对照组(P<0.05);2组治疗后SF-36生活质量量表中各项评分均较治疗前显著升高(P均<0.05),其中生理功能、生理职能、生命活力、精神健康、总体健康评分显著高于对照组(P均<0.05)。观察组治疗总有效率、治疗依从性的优良率显著高于对照组(P均<0.05),成本-效果比显著低于对照组(P<0.05)。结论中药配方颗粒制备通便膏方治疗慢性功能性便秘效果明显优于四磨汤,不仅可明显改善便秘症状,缓解不良情绪,提高生活质量,且治疗依从性更高,成本-效果比低,具有较好的药物经济学价值。
Objective It is to observe the curative effect of Tongbian herbal paste made with formula granule on chronic functional constipation and its effect on quality of life.Methods 120 patients with chronic functional constipation were randomly divided into control group(60 cases)and observation group(60 cases).The observation group was treated with Tongbian ointment prepared by traditional Chinese medicine formula granules,and the control group was treated with Simo decoction.Both groups were treated for 4 weeks.The changes of constipation symptom score,Wexner constipation score,mental state,Bristol stool type and quality of life were observed before and after treatment,and the clinical efficacy,treatment compliance and cost-effectiveness ratio were compared between the two groups.Results After treatment,the scores of self-defecation,defecation intention,defecation effort,defecation characteristics,defecation incompleteness,anal obstruction,dyspnea,defecation frequency,defecation time,anal distension,abdominal distension and total score of the two groups were significantly lower than those before treatment(P<0.05),the first seven scores and total score of the observation group were significantly lower than those of the control group(P<0.05).Wexner's constipation score,self-rating anxiety scale(SAS)and self-rating depression scale(SDS)scores in the two groups were significantly lower than those before treatment(P<0.05),the above scores in the observation group were signific antly lower than those in the control group(P<0.05).Bristol’s stool type was significantly improved after treatment in both groups(P<0.05),the improvement of the observation group was better than that of the control group(P<0.05).The scores of SF-36 quality of life scale in the two groups were significantly higher after treatment(P<0.05),and the scores of physiological function,physiological function,vitality,mental health and overall health were significantly higher than those in the control group(P<0.05).The total effective rate and the excellent and good rate of treatment compliance in the observation group were significantly higher than those in the control group(P<0.05),and the cost-effectiveness ratio was significantly lower than that in the control group(P<0.05).Conclusion Tongbian herbal paste prepared with formula granule is superior to Simo Decoction in treating chronic functional constipation.It can not only improve the symptoms of constipation,alleviate bad mood and improve the quality of life,but also has higher treatment compliance,lower cost-effectiveness ratio and better pharmacoeconomic value.
