摘要
目的观察动态监测重症肺炎致呼吸衰竭患者血液CRP(C-反应蛋白)、TNF-α(肿瘤坏死因子-α)、IL-18(白细胞介素18)水平变化的临床意义.方法选取本院收治的重症肺炎致呼吸衰竭患者40例,作为观察组,同时选取同时期行健康体检者30例,于治疗前后应用酶联免疫吸附试验(ELISA)以及散射比浊法测定CRP及TNF-α、IL-18指标变化,并对比两组治疗前后的血气分析指标.结果治疗前观察组的pH值及PO2水平明显低于对照组,PCO2水平明显高于对照组,治疗后观察组pH值及PO2水平明显升高(P<0.05),PCO2水平明显下降(P<0.05),治疗前观察组的CRP、TNF-α、IL-18水平均明显高于对照组,治疗后观察组CRP、TNF-α、IL-18水平均明显下降(P<0.05).结论检测及分析CRP、TNF-α、IL-18水平变化对于了解重症肺炎合并呼吸衰竭患者的病情及指导诊疗具有重要的临床意义.
Objective To observe the clinical significance of dynamic monitoring of blood CRP, TNF-α, IL-18 level for patients with severe pneumonia combine respiratory failure. Methods 40 cases patients of severe pneumonia were selected combine respiratory failure in our hospital as a group, and at the same 30 cases of health check-up were selected, used enzyme-linked immunosorbent assay (ELISA) and scattering turbidimetric method to determine the CRP and TNF alpha, IL-18 indicators before and after treatment, and compared the blood gas analysis index in two groups before and after treatment. Results Before treatment the pH and PO2 level of observation group were obviously lower than the control group, PCO2 levels were significantly higher than the control group and after treatment, pH and PO2 level increased significantly in observation group (P< 0.05), PCO2 levels significantly decreased (P<0.05),before treatment, CRP, TNF-α, IL-18 levels were significantly higher in the observation group than that of control group, after treatment, CRP, TNF-α, IL-18 levels significantly decreased in the observation group (P<0.05). Conclusion The detection and analysis of CRP, TNF alpha, IL-18 level changing for severe pneumonia combine respiratory failure patients can help us control the condition of condition and guide diagnosis and treatment.
