血小板与淋巴细胞比值预测肝硬化患者发生急性肾损伤的价值分析

摘要 目的探讨血小板与淋巴细胞比值在肝硬化患者发生急性肾损伤(AKI)的预测价值。方法选取于2016年1月-2017年12月本院感染、消化内科治疗的肝硬化住院患者206例作为研究对象,依据入院72 h内患者是否发生AKI(KDIGO标准)分为AKI组(60例)和非AKI组(146例),观察2组患者就诊初始血小板与淋巴细胞比值(PLR)、血胱s抑素C(Cys-C)、血乳酸(Lac)等指标变化。受试者工作曲线(ROC)分析上述指标对肝硬化并发AKI患者的诊断和预后效能。结果 ROC曲线分析显示,PLR预测AKI发生的AUC大于Cys-C和Lac,差异有统计学意义(P <0. 05);PLR预测AKI病情好转的AUC大于Cys-C,差异有统计学意义(P <0. 05)。结论 PLR是预测肝硬化患者并发AKI的早期、灵敏和特异的标志物,并有助于AKI患者病情评估。 Objective To assess the value of platelet to lymphocyte rate( PLR) in predicting acute kidney injury in patients with liver cirrhosis. Methods 206 cases with liver cirrhosis admitted to Department of Infectious Disease and Gastroenterology of our Hospital were enrolled in the research during January 2016 to December 2017. According to the KDIGO standard,liver cirrhosis patients were divided into AKI group( n = 60) and non-AKI group( n = 146). Platelet to lymphocyte rate was calculated,serum levels of Cys-C and Lac levels of NAG were detected on admission. Receiver operating characteristics( ROC)curve was performed to predict the validity of PLR,Cys-C and Lac. Results The area under ROC curve of PLR for predicting AKI was superior to Cys-C and Lac( P < 0. 05),the AUC value of PLR showed significantly higher than Cys-C in predicting the prognosis of liver cirrhosis related AKI patients. Conclusion PLR is an early,sensitive,specific clinical indicators for predicting AKI in patients with liver cirrhosis,and also can be used for dynamic evaluation of AKI prognosis.
