摘要
目的:观察并比较后腹腔镜与开放性手术对肾癌根治术患者康复效果和短期生存状态的影响。方法:选取2010年1月-2018年9月的80例肾癌根治术患者为研究对象,依据手术方式的不同分为A组(开放性手术组)和B组(后腹腔镜手术组),每组各40例。比较两组患者排气时间、排便时间、下床活动时间、术后住院时间、并发症发生率、术后1 d及3 d的疼痛程度评分、术前及术后1、4周的生存质量评分。结果:B组的排气时间、排便时间、下床活动时间及术后住院时间均短于A组,并发症发生率低于A组,术后1、3 d的疼痛程度评分均低于A组,差异有统计学意义(P<0.05),术前两组的生存质量评分比较,差异无统计学意义(P>0.05),术后1、4周B组的生存质量评分均高于A组,差异有统计学意义(P<0.05)。结论:后腹腔镜手术对肾癌根治术患者康复效果和短期生存状态的影响显著优于开放性手术,在肾癌根治术患者中具有相对更高的临床应用价值。
Objective:To observe and compare the influence of retroperitoneal laparoscopy and open surgery for the rehabilitation effect and short-term survival state of patients with radical nephrectomy for renal cancer.Method:80 patients with radical nephrectomy for renal cancer from January 2010 to September 2018 were chosen as the study objects,they were divided into group A(open surgery group)and group B(retroperitoneal laparoscopy group)according to the methods of surgery,40 cases in each group.The exhaust time,defecation time,ambulation time and postoperative hospitalization time,complications rates,pain degree scores at first and third day after the surgery,life quality scores before the surgery and at first,forth week after the surgery of two groups were compared.Result:The exhaust time,defecation time,ambulation time and postoperative hospitalization time of group B were shorter than those of group A,pain degree scores at first and third day after the surgery were all lower than those of group A,the differences of two groups were all statistically significant(P<0.05);the life quality scores of two groups before the surgery were compared,the differences of two groups were not statistically significant(P>0.05),the life quality scores of group B were all higher than those of group A,the differences of two groups were all statistically significant(P<0.05).Conclusion:The influence of retroperitoneal laparoscopy for the rehabilitation effect and short-term survival state of patients with radical nephrectomy for renal cancer are significantly better than those of open surgery,and its application value in patients with radical nephrectomy for renal cancer is relatively higher.
