摘要
目的:初步探索北京、上海妇女保健领域激励机制可落实程度,明确激励机制可落实程度对于实现妇女保健工作目标的相关性。方法:系统收集所有涉及京沪两地妇女保健领域公开相关政策文件,量化其可落实程度,运用Spearman秩相关和单因素线性回归模型分析可落实程度与孕产妇死亡率之间的关系。结果:2000~2017年京沪妇女保健领域激励机制可落实程度逐渐改善。2017年京沪差异不明显(16.16%vs17.91%)。两地的孕产妇死亡率与可落实程度呈负相关(P<0.01),上海可落实程度对孕产妇死亡率影响的解释程度为39%,北京为33.4%。结论:京沪逐步落实的激励机制对妇女健康结果的作用已初步显现;上海激励机制落实情况改善所发挥的作用略优于北京但两地对于激励机制的落实仍需加强;激励机制可落实程度的量化方法可尝试在其他公共卫生领域进一步验证。
Objective The significance of practicability of incentive mechanism of maternal care policies was revealed by analyzing the trend of practicability of incentive mechanism of maternal care policies and its differences in Beijing and Shanghai,and to discuss the feasibility of quantifying the practicability of maternal care of from the perspective of policy documents. Methods The public policy documents on maternal care in Beijing and Shanghai were collected to quantify the degree of implementation.Spearman rank correlation and univariate linear regression model were used to analyze the relationship between the degree and maternal mortality. Results The degree of implementation of incentive mechanism of Beijing and Shanghai had improved gradually from 2000 to 2017. In 2017,the difference between the two cities was not clear(16. 16% vs 17. 91%). The trend of the maternal mortality showed significant negative correlation to that of the degree of implementation in these two cities(P < 0. 01). The degree of Shanghai explained the impact of the maternal mortality rate was 39%,and Beijing was 33. 4%. Conclusion The inability to assess the responsibilities of the departments of maternal care in Beijing and Shanghai restricted the improvement of the implementation of incentive mechanism of these two cities. Defining the division of responsibilities of departments(institutions) clearly was the basis for effective implementation of incentive mechanism. It is feasible to comprehensively quantify the degree of implementation of incentive mechanism.
