摘要
目的探讨利多卡因气雾剂在桡骨远端骨折整复中的镇痛效果。方法选取2016年10月~2018年1月广州市正骨医院急诊科收治的120例桡骨远端骨折患者,随机分为A、B、C三组,每组40例。A组为空白对照,B组外喷利多卡因气雾剂镇痛,C组肌内注射酮咯酸氨丁三醇镇痛,比较三组不同时间点的疼痛评分(VAS)及血压、心率。结果B、C组整复前、整复时、整复后10min的VAS评分均低于A组,差异有统计学意义(P<0.05);B组整复时的VAS评分低于C组,差异有统计学意义(P<0.05);B、C组整复前、整复后10min的VAS评分比较,差异无统计学意义(P>0.05)。整复后,三组的血压、心率均较整复前升高,其中,A组整复前后的收缩压、舒张压、心率比较,差异有统计学意义(P<0.05);B组、C组整复前后的收缩压、舒张压、心率比较,差异无统计学意义(P>0.05)。B组、C组整复后的收缩压、舒张压、心率低于A组,差异有统计学意义(P<0.05);B组整复后的收缩压、舒张压、心率与C组比较,差异无统计学意义(P>0.05)。三组患者均未发生明显不良反应。结论利多卡因气雾剂应用于桡骨远端骨折整复镇痛的效果显著,使用方便,适合在门急诊骨折整复中推广应用。
Objective To investigate the analgesic effect of Lidocaine Aerosol in the reduction of distal radius fractures.Methods A total of 120 patients with distal radius fracture treated in emergency department of Guangzhou Orthopedic Hospital from October 2016 to January 2018 were selected and randomly divided into group A,group B and group C,with 40 people in each group.Group A was the blank group,Group B was given Lidocaine Aerosol for analgesia,and Group C was given Ketorolac Romethamine for analgesia.The pain score (VAS) and blood pressure and heart rate of the three groups at different time points were compared.Results The VAS score of group B and group C were lower than those of group A at the time points of before reduction,during reduction and 10 minutes after reduction,and the differences were statistically significant (P<0.05).The VAS score of group B was lower than that of group C at the time of during reduction,and the differences were statistically significant (P<0.05).There was no significant difference in the VAS scores in group B and group C between at the time of before reduction and 10 minutes after reduction (P>0.05).After reduction,the blood pressure and heart rate of the three groups were higher than that before reduction,among which,there were significant differences in systolic blood pressure,diastolic blood pressure and heart rate before and after reduction in group A (P<0.05).There was no significant difference in systolic blood pressure,diastolic blood pressure and heart rate in group B and group C between before and after reduction (P>0.05).The systolic blood pressure,diastolic blood pressure and heart rate in group B and group C after reduction were lower than those in group A,and the difference was statistically significant (P<0.05).There was no significant difference in systolic blood pressure,diastolic blood pressure,heart rate between group B and group C after reduction (P>0.05).There were no obvious adverse reactions in the three groups.Conclusion Lidocaine Aerosol has remarkable analgesic effect in the reduction of distal radius fractures.It is easy to use and suitable for the promotion and application in emergency and outpatient.
