摘要
目的分析脑瘫(CP)患儿视频脑电图(VEEG)结果。方法回顾性分析2017年4月~2018年8月于本院行VEEG的60例CP患儿临床资料,所有患儿均进行国际10/20系统电极放置,分析患儿脑电图异常分类、脑瘫类型与脑电图关系、合并癫痫患儿VEEG结果,并观察脑瘫合并癫痫发作类型。结果60例CP患儿中,脑电图异常者占47例,异常率为78.33%,以局部痫样放电占比最高(38.33%);痉挛型脑电图异常率为83.78%,不随意运动型脑电图异常率为44.44%,混合型脑电图异常率为85.71%,痉挛型与混合型脑电图异常率显著高于不随意运动型(P<0.05);21例脑瘫合并癫痫患儿中,以全面性发作占比最高(57.14%);合并癫痫患儿脑电图异常率95.24%,其中痫样放电发生率80.95%,显著高于未合并癫痫患儿的69.23%、53.95%(P<0.05)。结论CP患儿VEEG异常率高,通过VEEG异常特征,能辅助判断有无癫痫及发作类型,对CP患儿诊疗具有参考价值。
Objective To analyze the results of video-electroencephalogram (VEEG) in children with cerebral palsy (CP). Methods The clinical data of 60 children with CP who underwent VEEG in our hospital from April 2017 to August 2018 were retrospectively analyzed. All children were placed with international 10/20 system electrode. The abnormal classification of EEG, the relationship between cerebral palsy types and EEG, and the VEEG results of children with epilepsy were analyzed, and the types of cerebral palsy with epileptic seizures were observed. Results Among the 60 CP children, 47 cases were abnormal EEG, the abnormal rate was 78.33%, the proportion of partial epileptiform discharge was the highest (38.33%). The abnormal rate of spastic EEG was 83.78%, that of involuntary motor EEG was 44.44% and that of mixed EEG was 85.71%, the abnormal rate of spasm and mixed EEG were significantly higher than involuntary motor EEG (P < 0.05). Among 21 CP children with epilepsy, the proportion of comprehensive seizures was the highest (57.14%). The abnormal rate of EEG in children with epilepsy was 95.24%,and among them, the incidence rate of epileptiform discharge rate was 80.95%, they were significantly higher than those of the children without epilepsy( 69.23% and 53.95%)( P < 0.05). Conclusion The abnormal rate of VEEG in children with CP is high. The abnormal characteristics of VEEG can help to judge whether there is epilepsy or seizure type. It has reference value for the diagnosis and treatment of children with CP.
