摘要
目的探讨血清N末端脑钠肽前体(NT-proBNP)和降钙素原(PCT)在脓毒血症患者中的表达及临床意义。方法选取本院2016年10月至2018年9月诊治的脓毒血症患者113例,根据疾病严重程度分为感染组(n=82)和休克组(n=31),选取同期健康体检者113例为对照组,均行血清NT-proBNP和PCT检测。结果脓毒血症患者血清NT-proBNP(617.96±101.43)pg/mL、PCT(5.28±1.34)ng/mL均显著高于健康体检者(132.75±24.18)pg/mL、(0.17±0.06)ng/mL。休克组血清NT-proBNP(1278.46±92.13)pg/mL、PCT(10.81±1.08)ng/mL均显著高于感染组(368.26±15.87)pg/mL、(3.19±0.45)ng/mL。休克组死亡率(35.5%)显著高于感染组(1.2%)。差异有统计学意义(P<0.05)。血清NT-proBNP、PCT与脓毒血症呈明显正相关性(r=0.527,P=0.000)、(r=0.638,P=0.000)。结论血清NT-proBNP、PCT与脓毒血症的严重程度密切相关,可用于患者的预后评估,具有较高的临床价值。
Objective To investigate expression and clinical significance of serum NT-pro BNP,PCT in patients with sepsis.Methods 113 patients with sepsis were selected in hospital from October 2016 to September 2018,who were divided into infection group ( n =82) and shock group ( n =31) according to disease severity.113 healthy persons were selected at the same period as control group.Serum NT-pro BNP and PCT were detected.Results Serum NT-pro BNP(617.96±101.43)pg/mL,PCT(5.28±1.34)ng/mL in patients with sepsis were significantly higher than those(132.75±24.18)pg/mL,(0.17±0.06)ng/mL in healthy persons.Serum NT-pro BNP(1278.46±92.13)pg/mL,PCT(10.81±1.08) ng/mL in shock group were significantly higher than those(368.26±15.87)pg/mL,( 3.19±0.45)ng/mLin infection group.Mortality rate (35.5%)in shock group was significantly higher than that (1.2%) in infection group.Differences were statistically significant ( P <0.05).Serum NT-pro BNP and PCT were positively correlated with sepsis ( r =0.527,P =0.000),( r =0.638,P =0.000).Conclusion Serum NT-pro BNP and PCT are closely related to the severity of sepsis,which can be used to evaluate the prognosis of patients,which have high clinical value.
