乳腺癌HFUS图像与分子生物学指标的相关性研究 被引量:6

摘要 目的:探讨乳腺癌高频超声(HFUS)图像与相关生物学行为的关系。方法:选取2013年11月至2018年11月广东省人民医院南海医院收治的101例乳腺癌患者作为研究对象,分析比较不同HFUS超声征象(淋巴结转移、内部血流、微钙化、边界)乳腺癌患者的增殖细胞相关的核抗原(Ki-67)、原癌基因(CerbB-2)、孕激素受体(PR)、雌激素受体(ER)阳性率差异。结果:101例乳腺癌患者中,病变边界毛糙者91例、规整者10例,存在微钙化者57例、无微钙化者44例,内部血流丰富者71例、匮乏者30例,淋巴结转移者69例、无转移者32例。边界毛糙者ER阳性率(75.82%)高于规整者(40.00%)(P<0.05);微钙化者Ki-67阳性率(36.84%)、CerbB-2阳性率(26.32%)均低于无钙化者(65.91%、63.64%)(P<0.05);内部血流丰富者CerbB-2阳性率(49.30%)高于匮乏者(26.67%)(P<0.05);淋巴结转移者CerbB-2阳性率(28.99%)低于无转移者(71.88%)(P<0.05)。结论:乳腺癌HFUS影像学图像与Ki-67、CerbB-2、ER等生物学指标间存在一定关联性,可通过HFUS评估乳腺癌相关生物学行为,为临床治疗方案提供参考依据。 Objective: To investigate the relationship between high frequency ultrasound (HFUS) images of breast cancer and related biological behaviors. Methods: A total of 101 breast cancer patients admitted to our hospital from November 2013 to November 2018 were selected. The positive rates of proliferating cell-associated nuclear antigen (Ki-67), proto-oncogene (CerbB-2), progesterone receptor (PR) and estrogen receptor (ER) were detected and compared among patients with different HFUS ultrasound signs (lymph node metastasis, internal blood flow, microcalcification and border). Results: HFUS detected rough lesion boundary in 91 of 101 patients, microcalcifications in 57 patients, rich blood flow in 71 patients and lymphatic metastasis in 69 patients. Patients with rough lesion boundary had higher ER positive rate (75.82%) than the regular ones (40.00%)( P <0.05), and patients with microcalcification had lower positive rates of Ki-67 (36.84%) and CerbB-2 (26.32%) than those without (65.91% and 63.64%)( P <0.05). CerbB-2 positive rate (49.30%) was higher in patients with rich internal blood flow compared with patients who were deficient (26.67%)( P <0.05), but it was lower in patients with lymph node metastasis (28.99%)( P <0.05). Conclusion: There is a certain correlation between HFUS images of breast cancer and biological indicators such as Ki-67, CerbB-2 and ER. The biological behaviors of breast cancer can be evaluated by HFUS, which provides a reference for clinical treatment.
