摘要
目的探讨温湿化氧疗在脑卒中老年患者非人工气道管理中的应用效果.方法选择2016年6月~2018年6月在我院住院治疗的600例非人工气道脑卒中老年患者,随机分为观察组和对照组,每组各300例.观察组实施由文丘里面罩和MR410、MR760或MR810呼吸机湿化器、呼吸管路等组成的温湿化装置温湿化氧疗方案,对照组实施传统的鼻导管或面罩氧疗方案,观察两组的痰液黏稠度、痰痂形成、氧疗效果及肺部感染等情况.结果观察组的痰液黏稠度优于对照组(P<0.05);观察组的痰痂形成率、肺部感染发生率、动脉二氧化碳分压(PaCO2)均低于对照组,差异有统计学意义(P<0.05);观察组的血氧饱和度(SpO2)高于对照组,差异有统计学意义(P<0.05).结论文丘里温湿化氧疗装置可有效提高氧疗效果,湿化气道,降低肺部感染发生率,提高患者氧疗舒适度和依从性.
Objective To explore the application effect of warm humidification oxygen therapy in non-artificial airway management in elderly stroke patients. Methods A tatal of 600 elderly patients with non-artificial airway stroke who admitted to our hospital from June 2016 to June 2018 were selected. They were randomly divided into the observation group and the control group, 300 cases in each group. The observation group was provided with a warm humidifying oxygen therapy program consisting of a venturi mask and a humidifier of MR410, MR760 or MR810 or a breathing tube for the ventilator, and the control group was provided with a traditional nasal catheter or mask oxygen therapy program, the phlegm viscosity, sputum callus formation, oxygen therapy effect and pulmonary infection in the two groups were compared. Results The sputum consistency of the observation group was better than that of the control group (P<0.05). The sputum scab formation rate, pulmonary infection rate and arterial carbon dioxide partial pressure (PaCO2) in the observation group were all lower than those in the control group, and the differences were statistically significant (P< 0.05). The blood oxygen saturation (SpO2) of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). Conclusion Venturi warm humidifying oxygen therapy device can effectively improve the effect of oxygen therapy, humidifying the ainvay, reduce the incidence of pulmonary infection, and improve the comfort and compliance of patients with oxygen therapy.
