摘要
目的目前幼年特发性关节炎(juvenile idiopathic arthritis,JIA)发病率呈上升趋势,且预后不佳会严重危害儿童生命健康。本研究探讨JIA患儿的生活质量及其影响因素。方法选择2012-10-08-2016-10-08郑州大学附属儿童医院收治的84例JIA患儿为研究对象。随访2年,用儿童生活质量测定量表(pediatric quality of life inventory TM3.0-rheumatology module,PedsQLTM3.0)风湿模块对患儿生活质量进行评估,以PedsQLTM3.0评分<60分的26例患儿为研究组,PedsQLTM3.0评分≥60分的58例患儿为对照组。分析影响JIA患儿生活质量的相关因素,并进行Logistic多元回归分析。结果 Logistic多元回归分析结果显示,年龄≤5岁(OR=3.689,95%CI为1.073～12.681,P=0.038)、多关节型(OR=6.552,95%CI为1.545～27.782,P=0.011)、特应质(OR=2.815,95%CI为1.183～21.496,P=0.029)、社会支持少(OR=6.557,95%CI为1.617～26.590,P=0.008)、重度疼痛(OR=16.043,95%CI为2.682～95.968,P=0.002)是造成JIA患儿生活质量低下的独立危险因素。结论年龄≤5岁、多关节型、特应质、社会支持少和重度疼痛是造成JIA患儿生活质量低下的重要原因,临床上应予以相应的预防措施。
OBJECTIVE At present,the incidence of juvenile idiopathic arthritis(JIA) is on the rise,and the poor prognosis will seriously endanger children’s life and health.This paper will explore the quality of life and its influencing factors of children patients with JIA.METHODS 84 children patients with JIA admitted to Children’s Hospital Affiliated of Zhengzhou University from October 8,2012 to October 8,2016 were selected for the study.After 2 years of follow-up,the pediatric quality of life inventory TM3.0-rheumatology module(PedsQLTM 3.0) was used to evaluate the quality of life.26 cases with PedsQLTM 3.0 score<60 points were included in study group,and 58 cases of PedsQLTM 3.0 score≥60 points were included in control group.The related factors affecting the quality of life of children patients with JIA were recorded and given Logistic multivariate regression analysis.RESULTS The results of Logistic multivariate regression analysis showed that age≤5 years old(OR=3.689,95%CI:1.073-12.681,P=0.038),polyarticular type(OR=6.552,95%CI:1.545-27.782,P=0.011),atopy(OR=2.815,95%CI:1.183-21.496,P=0.029),low society support(OR=6.557,95%CI:1.617-26.590,P=0.006) and severe pain(OR=16.043,95%CI:2.682-95.968,P=0.002) were independent risk factor for low quality of life in children patients with JIA.CONCLUSIONS Age≤5 years old,polyarticular type,atopy,low social support and severe pain are important reasons for the low quality of life of children patients with JIA.Corresponding preventive measures should be given in clinical practice.
