摘要
目的 分析总结针刺治疗神经根型颈椎病的选经取穴规律。方法 以"针刺""神经根型颈椎病"等为主题词,检索6个中英文数据库,检索期限为2009~2018年,筛选出符合纳入标准的针刺治疗神经根型颈椎病的临床随机对照研究,以Microsoft Excel软件对其选经取穴规律进行总结分析。结果 最终纳入文献99篇,针刺治疗神经根型颈椎病:颈夹脊(79篇)、后溪(34篇)、风池(30篇)、肩井(25篇)、大椎(23篇)、外关(22篇)此6穴最为常用;常用穴主要集中在足少阳胆经(55篇)、手阳明大肠经(36篇)、手太阳小肠经(34篇)3条经脉;主穴主要集中于颈项部及上肢(161篇、92篇),交会穴的应用最多(78篇),其次是八脉交会穴(56篇)。结论 针刺治疗神经根型颈椎病取穴以相应节段颈夹脊结合足少阳胆经腧穴为主,注重颈部及上肢部相结合的远近配穴法,重视颈部交会穴与上肢八脉交会穴的应用。
Objective To summarize and analyze the law of point selection by acupuncture in treatment of nerve root type cervical spondylosis.Methods With acupuncture,cervical spondylosis,et al.as the keywords,literatures were searched from six Chinese-English databases,the date limit of which,was from 2009 to 2018.A randomized controlled trial of acupuncture for the treatment of cervical was screened,according to inclusion criteria and the law of point selection by acupuncture was summerized and analyzed.Results Finally,a total of 99 literatures were adopted.Six points were commonly used by acupuncture in the treatment of nerve root type cervical spondylosis,which were as follows: Neck Jiaji (79 papers),Houxi (SI 3)(34 papers),Fengchi (GB 20)(30 papers),Jianjing (GB 21)(25 papers),Dazhui (DU 14)(23 papers),Waiguan (SJ 5)(22 papers).The main points were mainly on the foot shaoyang gallbladder channel(55 papers),hand yangming large intestine channel(36 papers),and the hand taiyang small intestine channel (34 papers).And most of the main points were mainly on neck and upper limbs (161 papers and 92 papers) and the eight pulse convergent point(56 papers) were next to them.Conclusion Selecting acupoints by acupuncture in the treatment of nerve root type cervical spondylosis,were mainly along Neck Jiaji segment related,combined with foot shaoyang gallbladder channel.Far and near point combinations of neck and upper limb,were emphasised on.Meanwhile,convergent point of neck and upper limb and eight pulse were emphasised on,in clinical practice.
