CT能谱成像在甲状腺结节良恶性鉴别诊断中的应用价值 被引量:20

摘要 目的:研究CT能谱成像应用于甲状腺结节检出及在良恶性鉴别诊断中的价值。方法:拟行手术治疗的甲状腺结节患者40例为观察对象。将所有患者按照手术病理诊断结果的不同分为良性组24例与恶性组16例。对两组患者均进行CT能谱成像检查,对比两组不同时期碘浓度情况,不同时期能谱衰减曲线斜率情况,结节CT特征情况。同时,比较CT能谱成像诊断甲状腺结节的结果与病理诊断结果。结果:良性组与恶性组患者在平扫期、动脉期、静脉期碘浓度方面对比,前者更高(均P<0.05)。良性组与恶性组患者在平扫期、动脉期、静脉期能谱衰减曲线斜率方面对比,前者更低(均P<0.05)。良性组与恶性组患者在包膜、钙化、边界规则等方面对比均不明显(均P>0.05)。CT能谱成像诊断甲状腺结节的敏感度、特异度以及准确度与病理诊断对比差异无统计学意义(均P>0.05)。结论:甲状腺良恶性结节的甲状腺CT能谱成像碘浓度、能谱衰减曲线斜率在不同时期均存在明显差异,临床可通过CT能谱成像有效检出甲状腺结节,并在鉴别结节良恶性方面具有较高的价值。 Objective:To study and analyze the value of CT energy spectrum imaging in the detection of thyroid nodules and the differential diagnosis of benign and malignant.Methods:40 patients with thyroid nodules who planned to undergo surgical were recorded as subjects for observation.All patients were divided into benign group(24 cases)and malignant group(16 cases)according to the difference of surgical pathological diagnosis.CT energy spectrum imaging was performed on both groups,and iodine concentration,energy spectrum attenuation curve slope and CT characteristics of nodules were compared between the two groups at different periods.Meanwhile,the results of CT energy spectrum imaging in the diagnosis of thyroid nodules were compared with the pathological diagnosis.Results:The iodine concentration of the benign group was higher than the malignant group in the aspects of plain scan,arterial phase and venous phase(all P<0.05).Compared with patients in the benign group and the malignant group,the slope of the attenuation curve of energy spectrum in the scan phase,arterial phase and venous phase was lower in the former group(all P<0.05).Patients in the benign group and the malignant group were not significantly compared in terms of capsule,calcification,boundary rules,etc.(all P>0.05).The sensitivity,specificity and accuracy of CT energy spectrum imaging in the diagnosis of thyroid nodules were not significantly compared with the pathological diagnosis(all P>0.05).Conclusion:The iodine concentration and the slope of the attenuation curve of thyroid CT energy spectrum imaging of benign and malignant thyroid nodules are significantly different in different periods.In clinical practice,thyroid nodules can be effectively detected by CT energy spectrum imaging,which has a high value in identifying benign and malignant thyroid nodules.It is worthy of clinical application.
