摘要
目的探讨原发性颅内淋巴瘤(PIL)的核磁共振成像(MRI)诊断与鉴别诊断价值。方法选择2013年6月~2018年3月入本院诊断与治疗的经临床病理证实为PIL的患者30例作为研究对象,入组对象有完整资料,均接受MRI诊断,回顾性分析他们的资料,总结MRI诊断符合率,以及MRI诊断PIL征象等特点。结果本次研究30例PIL患者经MRI诊断符合26例,符合率为86.67%;30例PIL患者均为单发性肿瘤,包括左侧颞叶、左侧额叶、右侧小脑、胼胝体,MRI诊断各个部位符合率无显著差异(P> 0.05);MRI诊断信号特征包括TIW1多为等信号,低信号与混杂信号较少,而T2W1多为高信号,其余为混杂信号;增强扫描后均匀强化23例、不均匀强化7例,且24例出-现不同程度瘤周水肿;DWI序列扫描30例,病灶均显示不同程度弥散受限,瘤体ADC均值(0.67±0.307)×10^(-3)mm/s。结论 PIL采取MRI诊断有不错的诊断价值,可表现出一些特有的MRI特征,通过分析其表现可对大部分肿瘤准确判断,而且能定位肿瘤病灶部位,从而为手术治疗提供一定的参考依据。
Objective To explore the value of MRI in the diagnosis and differential diagnosis of primary intracranial lymphoma (PIL). Methods Thirty patients with PIL confirmed by clinical pathology who were admitted to our hospital for diagnosis and treatment from June 2013 to March 2018 were selected as the research objects. All the patients who were admitted to our hospital had complete data and underwent MRI diagnosis. Their data were analyzed retrospectively to summarize MRI diagnosis coincidence rate, the characteristics of PIL signs diagnosed by MRI and other characteristics. Results The MRI diagnosis of 30 PIL patients in this study conformed to 26 cases, with a coincidence rate of 86.67%. All 30 PIL patients were solitary tumors, including left temporal lobe, left frontal lobe, right cerebellum and corpus callosum. The coincidence rate of MRI diagnosis for each part was not significantly different (P > 0.05). MRI diagnostic signal features include: TIW1 is mostly equal signal, low signal and mixed signal are less. T2W1 is mostly high signal, and the rest is mixed signal. After contrastenhanced scanning, homogeneous enhancement occurred in 23 cases, heterogeneous enhancement in 7 cases, and peritumoral edema occurred in 24 cases to varying degrees. DWI sequence scan showed 30 cases with various degrees of diffusion limitation. ADC mean value of tumor body was(0.67±0.07)×10 -3mm/s. Conclusion MRI diagnosis of PIL has good diagnostic value and can show some special MRI features. Through analysis of its manifestations, most tumors can be accurately judged, and the tumor focus can be located, thus providing certain reference basis for surgical treatment.
