摘要
目的根据慢性阻塞性肺疾病(COPD)气道阻塞严重程度分级,比较各级炎症和代谢反应,验证肺功能、代谢及炎症反应的关系。方法 51例轻度至极重度COPD患者,轻、中度(GOLD1-2)n=21;重度(GOLD3)n=25,非常严重(GOLD 4)n=5。患者接受肺功能(肺活量测量)、代谢反应(甘油三酯(TG triglycerides)、总胆固醇(TC total cholestero)、高密度脂蛋白-胆固醇(HDL-C))和炎症反应(细胞因子:IL-6、IL-10、TNF-α(ELISA))的评估。结果本研究发现,GOLD3组患者与GOLD1-2患者相比,IL-10、甘油三酯浓度水平低,IL-6水平高。此外,GOLD1-2组TNF-α与HDL胆固醇呈负相关(P<0.05),GOLD 3组IL-6与IL-10呈负相关(P<0.05)、IL-6和总胆固醇(P<0.05)负相关,白细胞介素-10与高密度脂蛋白胆固醇负相关性(P=0.05)。结论重度COPD患者表现为IL-6浓度高、IL-10浓度低的炎症性反应状态。因此,在评估COPD严重程度时,IL-10可能是一种很有价值的细胞因子。
Objective To compare inflammatory and metabolic responses according to severity of airflow among patients with COPD and to verify the relationship between pulmonary function,metabolic and inflammatory profile.Methods 51 patients with mild to very severe COPD were recruited and divided according lung function,21 cases in the mild-moderate group(GOLD 1-2),25 cases in the severe group(GOLD 3)and 5 cases in the very severe group(GOLD 4).They were submitted to the assessment of lung function(spirometry),metabolic profile(triglycerides,total cholesterol,HDL-cholesterol)and inflammatory profile(cytokines:IL-6,IL-10,TNF-α(ELISA)).Results It found that the severe group had lower levels of IL-10,triglycerides,and higher IL-6 than the mild-moderate group did.Additionally,the mild-moderate group presented negative correlation between TNF-α and HDL cholesterol(P=0.05),and the severe group showed negative correlation between IL-6 and IL-10(P<0.05),IL-6 and total cholesterol(P<0.05)and negative correlation between IL-10 and HDL-cholesterol(P=0.05).Conclusion Our findings suggest that patients with severe COPD can exhibit compromised "inflammatory status",characterized by higher IL-6 and lower IL-10 concentration.Furthermore,IL-10 seems to be an interesting cytokine to be investigated in this kind of patients.
