摘要
目的:对比分析四腔心切面法和四腔心平面头侧偏斜法彩色多普勒超声检查对孕期胎儿心脏畸形的诊断价值。方法:回顾性分析3800例行产前超声检查的孕产妇临床资料。将采用四腔心切面法检查的1848例孕产妇纳入对照组,采用四腔心平面头侧偏斜法检查的1952例孕产妇纳入观察组。分析两种超声检查方法图像显示情况,并以产后新生儿查体或引产后尸检畸形种类为金标准,比较两种超声检查方法对胎儿心脏畸形诊断准确率。结果:观察组中获得满意心脏切面图像1918例(占98.26%),对照组中获得满意心脏切面图像1785例(占96.59%)。两组胎儿先天性心脏畸形共41例(占1.07%),其中观察组22例,对照组19例。观察组产前超声检出心脏畸形胎儿21例,诊断准确率为95.45%(21/22);对照组产前超声检出心脏畸形胎儿16例,诊断准确率为84.21%(16/19);观察组诊断准确率显著高于对照组,差异有统计学意义(x2=2.978,P<0.05)。结论:四腔心平面头侧偏斜法超声检查可以对四腔心、左室流出道以及右室流出道切面3个基本切面同时进行检查,可获得更优的心脏切面图像,克服单独四腔心切面法检查的不足,具有快速、安全以及灵敏度、特异度和准确率高等优点,可提高对胎儿心脏畸形的检出率。
Objective:To contrastively analyze the diagnostic values of four-chamber cardiac section and four-chamber cardiac plane cephalic deviation of color Doppler ultrasonography cardiac malformation of fetus during pregnancy.Methods:The clinical data of 3800 pregnant and lying-in women who underwent prenatal ultrasound examination were retrospectively analyzed.1848 pregnant and lying-in women who were examined by four-chamber cardiac section were divided into the control group,and 1952 pregnant and lying-in women who were examined by four-chamber cardiac plane cephalic deviation were divided into the observation group.And the types of postpartum neonatal physical examination or postpartum autopsy deformities were adopted as the gold standard.Through analyzed the displayed images of two ultrasonography,to compare the diagnostic accuracy of two methods for fetal cardiac malformation.Results:In observation group,1918 cases with the cardiac section images(98.26%)were satisfied,while 1785 cases(96.59%)with the cardiac section images were satisfied in control group.There were total 41 cases(1.07%),which included 22 cases in observation group and 19 cases in control group,were congenital heart malformations in both of two groups.In observation group,21 fetuses with cardiac malformations were found by prenatal ultrasound,and its diagnostic accuracy was 95.45%(21/22).And in control group,16 fetuses with cardiac malformations were found by prenatal ultrasound,and its diagnostic accuracy was 84.21%(16/19).In addition,the diagnostic accuracy of observation group was significantly higher than that of control group(x2=2.978,P<0.05).Conclusion:Four-chamber plane cephalic deviation echocardiography can simultaneously examine three basic sections include four-chamber heart,left ventricular outflow tract and right ventricular outflow tract.It can obtain better images of heart section and can overcome the shortcomings of single four-chamber plane echocardiography,and it has series of advantages include fast,safety,high sensitivity,high specificity and high accuracy,and it can increase the relevance rate of fetal cardiac malformation.
