摘要
目的:了解我院剖宫产围手术期抗菌药物使用情况,对其进行合理性评价,并对不合理用药提出解决对策。方法:采用回顾性调查法,收集我院2017年1–12月剖宫产病例,共计318份,从抗菌药物品种选择、围手术期抗菌药物使用时机、使用疗程、抗菌药物利用指数等方面进行分析。结果:318例患者围手术期均使用了抗菌药物,使用频率最高的是头孢唑啉(65.5%);围手术期术前0.5～1 h和夹脐带后(术中)给药分别有30例(9.4%)和249例(78.3%)。术后感染率为5.0%,其中以术后当日(12.5%)感染发生率最高;用药持续时间t≤48 h共计287例(90.2%);头孢唑啉和头孢唑肟的药物利用指数> 1;剖宫产手术预防不合理用药有290例(91.2%)。结论:我院剖宫产围手术期抗菌药物使用存在多处不合理,主要体现在预防用药剂量过大、品种选择不合理、疗程过长等,需要进一步有针对性的加强监管。
Objective:To investigate the perioperative use of antibacterials for cesarean section in our hospital in order to evaluate the rationality of drug use and put forward improvement proposals.Methods:A total of 318 cesarean section cases from January 2017 to December 2017 in our hospital were retrospectively collected and analyzed in terms of the choice of antibacterials,medication time,course of medication and drug utilization index.Results:Antibacterials were used in all the 318 cesarean section cases,in which cefazolin was mostly used(65.5%).About 30 cases(9.4%)were given antibacterials 0.5– 1 hours before operation while 249 cases(78.3%)were given antibacterials after clamping the umbilical cord.About 5.0%of patients developed postoperative infection,in which most patients(12.5%)developed postoperative infection on operation day.The medication duration of 287 cases(90.2%)was less than or equal to 48 h.Drug utilization indexes of cefazolin and ceftizoxime were all more than 1.Antibacterials were irrationally used in 290 cases(91.2%).Conclusion:There were many problems in perioperative use of antibacterials for cesarean section in our hospital including excessive dosage,unreasonable selection of antibacterials and long course of medication.Targeted management measures should be furtherly made to improve rational drug use.
