摘要
目的评价实时荧光定量聚合酶链反应(PCR)在研究人乳头瘤病毒(HPV)DNA与宫颈癌相关性中的临床应用价值,探讨HPV与宫颈癌的相关性。方法选取经组织活检确诊的宫颈癌、宫颈糜烂患者及宫颈组织正常者共125例,采集其宫颈组织并进行HPV DNA实时荧光定量PCR检测,观察HPV DNA的检出率和病毒水平,分析其与宫颈癌临床分期的相关性。结果 60例宫颈癌组织中检出HPV 16、18型DNA检出54例,检出率为90.00%,DNA模板平均拷贝数为4.83×105 copy/mL,其中HPV 16、18型DNA均阳性者8例,仅检出HPV 18型DNA阳性者6例,仅检出HPV 16型DNA阳性者32例。35例宫颈糜烂组织中检出5例HPV 16型DNA阳性,检出率为14.29%,DNA模板平均拷贝数为5.85×103 copy/mL,未检出HPV 18型DNA阳性者。30例宫颈正常组织未检出HPV 16型及HPV 18型DNA阳性。对不同组别(宫颈癌组、宫颈糜烂组、宫颈正常组)的HPV DNA平均拷贝数比较,差异有统计学意义(P<0.05),且随组织病变情况加重,具有明显相关性(r=0.88)。结论 HPV DNA水平与宫颈癌的发生有相关性,且HPV DNA拷贝数随病情加重而增加,测定HPV DNA水平对宫颈癌的筛查有一定临床意义。
Objective To evaluate the clinical value of real-time fluorescence quantitative polymerase chain reaction(PCR)in studying the correlation between human papillomavirus(HPV)DNA and cervical cancer,and to explore the correlation between HPV level and cervical cancer.Methods A total of125cases of cervical cancer,cervical erosion and normal cervical tissues were selected.The cervical tissues were collected and detected by real-time fluorescent quantitative PCR.The detection rate and viral level of HPV DNA were observed,and the correlation between HPV DNA and clinical stages of cervical cancer was analyzed.Results In60cases of cervical cancer,54cases of16and18subtypes HPV DNA were detected,the detection rate was90.00%.The average copy number of DNA template was4.83×10^5copy/mL.Among them,8cases were detected for16and18subtype HPV DNA,only6cases were positive for18subtype HPV DNA,and only32cases were detected for HPV16.5cases of HPV16DNA were detected in35cases of cervical erosion.The detection rate was14.29%.The average copy number of DNA template was5.85×10^3copy/mL.No HPV18DNA was detected.HPV16and HPV18DNA in30cases of normal cervical tissue were not detected.The average copies of HPV DNA in different groups(cervical cancer group,cervical erosion group,normal tissue group)were compared,and the difference was statistically significant(P<0.05),and they were correlated with the aggravation of pathological changes(r=0.88).Conclusion The expression rate and expression level of HPV DNA are increased with the development of cervical cancer.Moreover,its expression level is positively correlated with the development of cervical cancer.
