摘要
目的探讨右美托咪定对手术切除肺癌单肺通气患者黄嘌呤氧化酶(XOD)、髓过氧化物酶(MPO)表达水平及中性粒细胞(PMN)计数的影响。方法选取本院90例手术切除肺癌单肺通气患者,根据麻醉方法的不同分为C组与D组各45例,C组麻醉诱导前给予常规生理盐水静脉注射,D组麻醉诱导前给予右美托咪定静脉注射,两组各45例。通过ELISA法对两组患者麻醉诱导前(T1)、单肺通气前即刻(T2)、单肺通气1 h(T3)、膨肺后1 h (T4)及术后1 d(T5)时的血清XOD及MPO表达水平进行测定,并对两组PMN计数进行测定,且比较两组手术时间、术中出血量、单肺通气时间及苏醒时间等情况。结果两组在手术时间、单肺通气时间、术中出血量、补液量及尿量比较,均无统计学意义(均P> 0. 05);两组T3—T5时XOD、MPO表达水平及PMN计数均高于T1(均P <0. 05),且D组各指标均显著低于C组(均P <0. 05); D组苏醒时间显著长于C组(P <0. 05)。结论右美托咪定能够明显减少手术切除肺癌单肺通气患者PMN计数,降低XOD与MPO的表达水平,有助于减轻患者肺组织缺血-再灌注损伤。
Objective To investigate the effect of dexmedetomidine on the expression of xanthine oxidase(XOD)and myeloperoxidase(MPO)and polymorphonuclear neutrophil(PMN)counts in patients with one-lung ventilation undergoing surgical resection of lung cancer.Methods 90 patients with one-lung ventilation undergoing surgical resection of lung cancer were divided into the group C and the group D of 45 patients in each group according to different anesthesia methods.The group C was given saline intravenously for intravenous injection before induction and the group D received dexmedetomidine for intravenous injection before induction.The expression of serum XOD and MPO were determined by ELISA in the two groups before induction of anesthesia(T1),immediately before one-lung ventilation(T2),1 h of one-lung ventilation(T3),lung inflation(T4)and 1 h after operation(T5),and the PMN counts were measured too.The operation time,intraoperative bleeding loss,one-lung ventilation time,recovery time and other indexes were compared between the two groups.Results There was no significant difference in operation time,one-lung ventilation time,intraoperative bleeding loss,fluid volume or urine volume between the two groups.The expression levels of serum XOD,MPO and PMN counts were were significantly higher at T2-T5 than at T1(all P<0.05),and those indexes were significantly lower in the group D than in the group C(all P<0.05).The recovery time in the group D was significantly longer than that in the group C(P<0.05).Conclusion Dexmedetomidine can significantly reduce the PMN counts and decrease the expression levels of XOD and MPO in patients with one-lung ventilation undergoing surgical resection of lung cancer,which can help to alleviate the ischemia-reperfusion injury in lung tissues.
