摘要
目的观察卡培他滨节拍化疗维持治疗经卡培他滨联合化疗治疗有效的晚期三阴性乳腺癌患者的效果和安全性。方法将经卡培他滨联合化疗方案治疗有效的66例浸润性三阴性乳腺癌患者随机分为小剂量组22例、常规剂量组23例和未维持组21例,分别给予小剂量卡培他滨节拍化疗、常规剂量卡培他滨化疗及不给予特殊治疗。4个周期后观察3组患者近期疗效及药物不良反应。结果小剂量组、常规剂量组的客观缓解率(ORR)、疾病控制率(DCR)高于未维持组(P <0. 05),小剂量组与常规剂量组的ORR、DCR比较,差异均无统计学意义(均P> 0. 05)。小剂量组未出现Ⅲ～Ⅳ级的骨髓抑制、消化道反应、手足综合征等不良反应,常规剂量组分别出现4例、6例、7例。结论对经联合化疗有效的晚期三阴性乳腺癌患者给予卡培他滨维持治疗近期疗效较好,小剂量节拍化疗治疗效果与常规剂量相当,但毒副反应更低。
Objective To observe the efficacy and safety of metronomic maintenance using capecitabine as maintenance therapy for advanced triple-negative breast cancer who benefit from the combined chemotherapy using capecitabine.Methods Sixty-six patients with invasive triple-negative breast cancer,benefited from the combined chemotherapy using capecitabine,were randomly divided into low dose group(n=22),conventional dose group(n=23)and non-maintenance group(n=21).The low dose group and conventional dose group received metronomic chemotherapy using low dose and conventional dose of capecitabine respectively,and the non-maintenance group didn′t receive any special treatment.The short-term efficacy and adverse drug reactions were observed among the three groups after four cycles of treatment.Results The objective response rate(ORR)and disease control rate(DCR)in the low dose group and conventional dose group were higher than those in the non-maintenance group(P<0.05),and no statistically significant differences were found in the ORR or DCR between the low dose group and the conventional dose group(all P>0.05).No cases of gradeⅢ-Ⅳmyelosuppression,gastrointestinal reactions or hand-foot syndrome occurred in the low dose group,while 4 case,6 cases and 7 cases,respectively,occurred in the conventional dose group.Conclusion Maintenance therapy using capecitabine achieves a favorable short-term efficacy for patients with advanced triple-negative breast cancer who benefit from the combined chemotherapy.The low-dose metronomic chemotherapy,with lower toxicity,has a similar therapeutic effect with the conventional therapy.
