摘要
目的:研究Y-V成形术联合糖皮质激素及A型肉毒毒素注射治疗瘢痕挛缩的效果,分析连续的Y-V成形术应用于瘢痕挛缩的原理。方法:本组共46例瘢痕挛缩患者,随机分为观察组和对照组,每组23例,对照组采用单纯的连续Y-V成形术治疗,观察组采用连续的Y-V成形术联合糖皮质激素及A型肉毒毒素局部注射治疗,比较两组临床疗效及不良反应发生情况。应用数学模型阐述连续的Y-V成形术治疗挛缩瘢痕的基本原理,用三角函数论证相关参数的关系,分析影响瘢痕挛缩治疗效果的因素。结果:观察组总有效率为86.9%高于对照组的56.5%,且观察组术后关节活动度改善度为(9.48±3.34)°高于对照组的(7.43±1.49)°,差异均有统计学意义(P<0.05)。两组间不良反应发生率比较差异无统计学意义(P>0.05)。结论:Y-V成形术联合糖皮质激素及A型肉毒毒素注射治疗瘢痕挛缩效果显著,操作简单,并发症少,值得推广应用。
Objective To study the effect of continuous Y-V plasty combined with glucocorticoid and botulinum toxin type A in the treatment of scar contracture.And to analyze the principle of continuous Y-V plasty in the treatment of scar contracture.Methods A total of 46 patients with scar contracture were randomly divided into the observation group and the control group,23 cases in each group.The control group was treated with continuous Y-V plasty,and the observation group was treated with continuous Y-V plasty combined with glucocorticoid and botulinum toxin type A.The clinical efficacy and adverse reactions were compared between the two groups.The basic principle of continuous Y-V plasty for cicatricial contracture is expounded by using mathematical model.The relationship of related parameters is demonstrated by trigonometric function,and the factors influencing the therapeutic effect of cicatricial contracture are analyzed.Results The total effective rate of the observation group was 86.9%,which was higher than that of the control group 56.5%,and the improvement of joint activity in the observation group was(9.48±3.34)°,which was higher than that in the control group(7.43±1.49)°,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Y-V plasty combined with injection of glucocorticoid and botulinum toxin type A for cicatricial contracture has obvious effect,simple operation and fewer complications.It is worth popularizing and applying.
