摘要
目的:探讨MRI弥散加权成像在骨肌肿瘤诊断中的应用;方法:将2015年3月-2018年3月笔者所在医院收治的80例骨肌肿瘤患者作为研究对象,患者均行活检病理或手术病理证实,其中29例为恶性肿瘤,51例为良性肿瘤。观察和分析不同B值时表观弥散系数值(ADC)的情况。结果:良性肿瘤在600、800、1 000 s/mm2下,ADC值高于恶性肿瘤、正常肌肉ADC值,比较差异有统计学意义(P<0.05)。良性肿瘤在400 s/mm2下ADC值高于恶性肿瘤、正常肌肉ADC值,比较差异有统计学意义(P<0.05);良性肿瘤、正常肌肉在400 s/mm2下ADC值比较差异无统计学意义(P>0.05);骨肌良恶性肿瘤在病灶直径、边界、T1信号、T2信号比较差异无统计学意义(P>0.05);骨肌良恶性肿瘤在周围水肿、坏死间比较差异有统计学意义(P<0.05)。结论:MRI弥散加权成像对诊断和鉴别骨肌良恶性肿瘤具有重要价值,值得临床选择。
Objective:To discuss the application of MRI-DWI(Magnetic Resonance Imaging-Diffusion Weighted Imaging)in musculoskeletal tumor.Method:A total of 80 patients with musculoskeletal tumor treated from March 2015 to March 2018 in our hospital were selected.Through the biopsy or operation,there were 29 malignant tumor cases and 51 benign tumor cases.The apparent diffusion coefficients(ADC)with different B values were analyzed.Result:At 600,800 and 1 000 s/mm2,the ADC in the benign tumor were higher than those in the malignant tumor and normal tissues,had significant difference among groups(P<0.05).At 400s/mm2,the ADC in the benign tumor were significantly higher than those in the malignant tumor and normal tissues(P<0.05).At 400s/mm2,the ADC in the benign tumor and normal tissues were not significantly different(P>0.05).the tumor diameter,boundary,T1 signals and T2 signals in the malignant and benign tumor were not significantly different(P>0.05).Conclusion:The MRI-DWI is of great importance in the diagnosis and differential diagnosis of malignant and benign musculoskeletal tumor.It shall be applied in clinical practice.
