从卵泡凋亡的调节机制研究左归丸对免疫性卵巢早衰的治疗作用 被引量:11

摘要 目的:研究卵泡调节机制中左归丸对免疫性卵巢早衰患者的临床效果。方法:研究纳入雌性小鼠,抗原以小鼠透明带多肽ZP3为主,皮下多点注射免疫BALB/C建立免疫性卵巢早衰模型。根据左归丸治疗剂量不同分为低剂量、中剂量及高剂量三组,同时以己烯雌酚作为阳性对照组;分别采用流式细胞技术检测小鼠外周血CD3^+、CD4^+、CD8^+、NK水平,以酶联免疫法(ELISA)检测小鼠外周血清FSH、E2及AZAb值,端口末端标记法(Tunel)检测小鼠颗粒细胞凋亡率,免疫组化法检测小鼠卵巢Fas-L、Bcl-2蛋白的表达,通过电镜观察小鼠卵巢组织的微观变化。结果:左归丸低剂量组、中剂量组、高剂量组及己烯雌酚组FSH水平均显著低于模型组,其中以低剂量组和高剂量组下降明显,差异显著,P<0.05;左归丸低剂量组、中剂量组及高剂量组E2水平均显著升高,差异显著,P<0.05。各组间CD4^+、CD8^+、CD4^+/CD8^+数值水平变化不明显,差异无意义,P>0.05;左归丸用药三组NK细胞均明显低于模型组,尤以左归丸低剂量组减少明显,差异显著,P<0.05。左归丸三组和己烯雌酚组AZAb值均明显低于模型组,以左归丸高剂量组最明显,差异显著,P<0.05;左归丸各剂量组AZAb值均明显低于己烯雌酚组,差异显著,P<0.05;左归丸三组和己烯雌酚组细胞凋亡率均明显低于模型组,差异显著,P<0.05。左归丸中剂量组、高剂量组及己烯雌酚组Bcl-2蛋白均明显高于模型组,以左归丸中剂量组最明显,差异显著,P<0.05;左归丸三组Fas-L蛋白均明显升高,差异显著,P<0.05。结论:左归丸治疗免疫性卵巢早衰主要通过调节免疫平衡、性腺内分泌水平、卵泡凋亡率及卵巢组织微观结构等方面有效改善卵巢功能,达到治疗卵巢早衰的目的,为临床维护和防治卵巢功能提供一定的基础依据。 Objective:To study the effects of Zuogui Wan(左归丸)on the regulation mechanism of follicular apoptosis in patients with premature ovarian failure.Methods:The female mice were given subcutaneous injection of zona pellucida pepitide 3,to establish mice model of autoimmune premature ovarian failure.According to the different dose of Zuogui Wan,female mice were divided into low dose,middle dose and high dose group;then the positive control group was given diethylstilbestrol.The levels of CD3+,CD4+,CD8+and NK in peripheral blood were detected respectively by flow cytometry;FSH,E2 and AZAb values were detected by ELISA;the apoptotic rate of mouse granulosa cells was detected by Tunel;the expression of Fas-L and Bcl-2 protein was detected by immunohistochemistry.Results:The FSH levels in the Zuogui Wan low dose group,middle dose group,high dose group and diethylstilbestrol group were significantly lower than the model group;that in the Zuogui Wan low dose group and high dose group was significantly decreased,P<0.05.The levels of E2 in the Zuogui Wan low dose group,middle dose group and high dose group were significantly increased,with a significant difference,P<0.05.There was no significant difference in CD4+,CD8+and CD4+/CD8+between different groups,P>0.05;The NK cells in the Zuogui Wan groups were significantly lower than the model group,especially in the low dose group,with significant differences,P<0.05.The AZAb values in the Zuogui Wan groups and diethylstilbestrol group were significantly lower than the model group,especially in the high dose group,with significant differences,P<0.05;and AZAb values in the Zuogui Wan groups were lower than the model group,with significant differences,P<0.05.The apoptosis rate in the Zuogui Wan groups and diethylstilbestrol group were significantly lower than the model group,with significant differences,P<0.05.The Bcl-2 protein in the Zuogui Wan middle dose group,high dose group and diethylstilbestrol group were significantly higher than the model group,especially in the middle dose group with significant differences,P<0.05.The Fas-L protein in the Zuogui Wan groups were significantly increased;the difference was significant,P<0.05.Conclusion:Zuogui Wan on autoimmune premature ovarian failure shows a better effect,mainly through regulating immune balance,gonadal hormone levels,follicle apoptosis rate and ovarian tissue microstructure,to improve ovarian function,can provide basis for clinical prevention and maintenance of ovarian function.
