摘要
目的:探讨精神分裂症复诊患者病耻感与生活质量的关系。方法:以我院125例处于康复期精神分裂症患者为研究对象,在患者出院前、出院1个月复诊时分别采用Link病耻感系列量表(贬低-歧视感知量表、病耻感应对量表、病耻感情感体验量表)评估其病耻感,采用精神分裂症患者生活质量量表评估其生活质量。并分析病耻感与生活质量的相关性。结果:研究对象出院前病耻感贬低-歧视感知、保密、退缩、教育、挑战、分离、误解、不同/羞耻等评分依次为(2.42±0.41)分、(2.72±0.45)分、(2.65±0.34)分、(2.75±0.41)分、(2.94±0.42)分、(2.70±0.36)分、(2.76±0.58)分、(2.32±0.45)分,均显著高于复诊时评分(2.07±0.38)分、(2.26±0.41)分、(2.12±0.15)分、(2.31±0.34)分、(2.42±0.36)分、(2.15±0.32)分、(2.27±0.34)分、(2.15±0.28)分(P<0.05);研究对象生活质量总评分(22.3±9.6)分,显著低于复诊时总评分(35.5±12.8)分(P<0.05);其病耻感与生活质量(心理社会评分、精力/动机评分)呈正相关(r=0.24,r=0.25,P<0.05),症状/副反应评分与病耻感无显著相关(r=0.14,P>0.05)。结论:精神分裂症复诊患者病耻感与生活质量呈正相关,患者病耻感强,其生活质量差,病耻感弱,其生活质量高。
Objective:To investigate the relationship between stigma and quality of life of subsequent visit patient with schizophrenia.Methods:125 patients with convalescent schizophrenia in the hospital were selected as the subjects.Their stigma was evaluated with the Link stigma scales(perceived devaluation discrimination scale,stigma coping scale,stigma-related feelings scale)before discharge and at subsequent visit at 1 month after discharge.The quality of life was assessed with the quality of life scale of patients with schizophrenia.The correlation between stigma and quality of life was analyzed.Results:Scores of stigma perceived devaluation discrimination,confidentiality,withdrawal,education,challenge,separation,misunderstanding and difference/shame before discharge[(2.42±0.41),(2.72±0.45),(2.65±0.34),(2.94±0.42),(2.70±0.36),(2.76±0.58),(2.32±0.45)]were significantly higher than those at subsequent visit[(2.07±0.38),(2.26±0.41),(2.12±0.15),(2.31±0.34),(2.42±0.36),(2.15±0.32),(2.27±0.34),(2.15±0.28)](P<0.05).The total score of quality of life(22.3±9.6)was significantly lower than that at subsequent visit(35.5±12.8)(P<0.05).The stigma was positively correlated with quality of life(psychological and social score,energy/motivation score)(r=0.24,r=0.25,P<0.05),but there was no significant correlation between symptoms/side effects score and stigma(r=0.14,P>0.05).Conclusion:The stigma of subsequent visit patients with schizophrenia is positively correlated with quality of life.The stigma is weak,the quality of life is high.
