摘要
目的:探讨大Y染色体核型在流产、死胎等不良妊娠结局中的生物、临床意义。方法:采用病例对照分析法,收集2010-02~2017-04在内蒙古医科大学第一附属医院就诊或咨询的2252对存在不良妊娠结局史的育龄夫妇纳入研究,实验人群均为男性,采用常规外周血细胞培养及染色体标本制备、G显带核型分析,同时将同期920对无流产史、曾足月分娩的正常夫妇纳入对照组,比较不同组别间大Y检出率的统计学差异,并探讨大Y染色体的临床意义。结果:不良妊娠组2252例受检标本中,共检出833例大Y染色体核型,检出率为36.99%;对照组920例受检标本中,检出大Y核型97例,检出率为10.54%,组间差异有统计学意义(P<0.05);不良妊娠组中大Y核型检出者具有临床效应者(少弱畸精症、无精症、睾丸或附睾发育不良等)为289例,占34.69%,对照组中大Y核型检出者中具有临床效应者为5例,占5.15%,两组间差异有统计学意义(P<0.05)。结论:大Y染色体核型在遗传生殖方面存在一定的临床效应,在流产、死胎等不良妊娠结局发生因素中具有临床意义。
Objective:To investigate the biological and clinical significance of large Y chromosome nuclear type in the end of miscarriage and fetal death.Methods:Through case study and comparison,collecting 2010-02~2017-04 in Inner Mongolia medical university first affiliated hospital or consulting of 2252 couples of childbearing age,which is in the history of adverse pregnancy outcomes included in the study,experimental group as the object of male,the conventional peripheral blood cell culture and chromosome specimen preparation,G banding karyotype analysis,and there is no history of miscarriage,will be in the same period of 920 to normal once term couple into the control group,comparing each big Y detection rate statistically,and to explore the clinical significance of the big Y chromosome.Results:In 2252 specimens,833 cases of large Y chromosome were detected and 36.99%were detected.In the control group 920 specimens,97 were detected in the large Y nuclear type,the detection rate was 10.54%,and the difference in the group was statistically significant(P<0.05).Adverse pregnancy group cuhk Y karyotype detection with clinical effect for the(little weak abnormal disease,no refined disease,testis and epididymis dysplasia,etc.)for 289 cases,accounting for 34.69%,control group cuhk Y karyotype detection with clinical effect in for 5 cases,accounted for 5.15%,the difference between the two groups was statistically significant(P<0.05).Conclusion:big Y chromosome karyotype in genetic reproduction of certain clinical effect,such as abortion,stillbirth,bad pregnancies occur factors have clinical significance.
