摘要
目的探讨尿微量白蛋白肌酐比、超敏C-反应蛋白(high sensitive C-reactive protein,hs-CRP)、同型半胱氨酸(homocysteine,HCY)、糖化血红蛋白(glycosylated hemoglobin,Hb A1c)和肾功能指标在2型糖尿病(type 2 diabetes mellitus,T2DM)患者及糖尿病肾损伤患者中的诊断意义。方法收集2013年1月~2015年11月期间在本院体检的2型糖尿病患者、2型糖尿病合并肾损伤的患者及健康体检者的生化检测结果,并进行整理分析。结果与健康体检者相比,糖尿病组和糖尿病肾损伤组各项检测指标水平有一定程度升高,其中糖尿病对照组各项检测指标差异均无统计学意义,糖尿病合并肾损伤组各项检测中尿微量白蛋白肌酐比、同型半胱氨酸、超敏C反应蛋白及尿素差异均有统计学意义;与糖尿病组相比,糖尿病合并肾损伤组各项指标有一定程度的升高,其中尿微量白蛋白肌酐比、同型半胱氨酸、超敏C反应蛋白及尿素差异均有统计学意义(P<0.05)。而患者的尿微量白蛋白肌酐比和同型半胱氨酸与糖尿病患者肾功能的损伤具有正相关性。结论尿微量白蛋白肌酐比、同型半胱氨酸、超敏C反应蛋白及尿素检测对糖尿病肾损伤的早期诊断具有重要意义,可及早评价患者的肾功能,减轻患者痛苦。
Objective Investigate the diagnostic significance of the urinary albumin creatinine ratio,high sensitive C-reactive protein(hs-CRP),homocysteine(HCY),glycosylated hemoglobin(HbA1c)and renal function in type 2 diabetes mellitus(T2DM)patients and diabetic patients with renal injury.Methods Conducte the biochemical tests of type 2 diabetes mellitus patients,type 2 diabetes mellitus patients with kidney injury and physical examination in our hospital during the period of January 2013 to November 2015,and analyze the results.Results Compared with the healthy subjects,the levels of the indexes in the diabetic group and diabetic kidney injury group were increased to some extent,and the differences of the indexes in the diabetic control group were not statistically significant,The levels of urinary microalbumin creatinine ratio,HCY,hs-CRP and urea were significantly different among the diabetes mellitus complicated with renal injury,Compared with the diabetic group,the index indiabetic kidney injury group increase to a certain extent,the urinary albumin creatinine ratio,HCY,hs-CRP and urea had significant differences(P<0.05).The patient's urinary albumin creatinine ratio and HCY were positively related to the impairment of renal function in patients with diabetes mellitus.Conclusion Urinary albumincreatinine ratio,HCY,hs-CRP and urea are important for early diagnosis of diabetic kidney injury,which can be used to evaluate the renal function and alleviate the pain of patients.
