摘要
目的 研究加味固冲汤联合西药对功能失调性子宫出血（DUB）患者雌激素分泌及凝血功能的影响。方法选取本院收治的DUB患者患者90例，随机分为观察组与对照组，每组各45例。对照组采用地屈孕酮治疗，观察组在对照组治疗的基础上加用加味固冲汤治疗，2组治疗周期均为12周。比较2组患者临床疗效；统计治疗前后2组患者月经期平均出血时间、子宫内膜厚度、雌激素水平及凝血功能指标；统计2组不良反应发生率。结果治疗后，观察组总有效率为91.11%，显著高于对照组的73.33%（P<0.05）；与治疗前相比，治疗后2组患者月经期平均出血时间及子宫内膜厚度均显著下降（P<0.01），且观察组明显低于对照组（P<0.05或P<0.01）；与治疗前相比，治疗后2组患者血清FSH、LH、P水平均显著下降，且观察组明显低于对照组（P<0.01）；治疗前相比，治疗后2组患者PT、APTT、TT均显著缩短（P<0.01），且观察组明显短于对照组（P<0.05或P<0.01），2组FIB水平均显著下降，且观察组明显低于对照组（P<0.01）；观察组不良反应发生率6.67%显著低于对照组的26.67%（P<0.05）。结论加味固冲汤联合西药治疗DUB可显著控制患者出血症状，降低其子宫内膜厚度，改善雌激素分泌异常，促进凝血功能恢复正常，且不良反应较少，临床疗效优于单用西药治疗。
Objective To explore the influence of jiawei guchong decoction combined with western medicine on the estrogen secretion and the coagulation function of patients with dysfunctional uterine bleeding(DUB).Methods90cases of patients with DUB in our hospital were selected and divided randomly into observation group and control group,each group was for45cases.Control group were treated with progesterone,and observation group were treated with jiawei guchong decoction on the basis of control group,2groups were treated for12weeks.Compared the clinical efficacy of2groups,statisticsed the mean bleeding time,endometrial thickness,the level of estrogen secretion and coagulation function indexes of2groups before and after treatment;statisticsed the incidence of adverse reaction of2groups.Results After treatment,the total effective rates of observation group was91.11%,which was obviously higher than that73.33%in control group(P<0.05);Compared with before treatment,the mean bleeding time and endometrial thickness of2groups decreased obviously(P<0.01),and observation group was obviously lower than control group(P<0.05or P<0.01),compared with before treatment,the serum FSH,LH,P,levels of2groups decreased obviously,and observation group was obviously lower than control group(P<0.01);compared with before treatment,the PT,APTT,TT of2groups shortened significantly after treatment(P<0.01),and observation group was obviously shorter than control group(P<0.05or P<0.01),the FIB level of2groups decreased obviously,and observation group was obviously lower than control group(P<0.01);the incidence of adverse reaction of observation group was6.67%,which was obviously higher than that26.67%in control group(P<0.01).Conclusion Guchong jiawei decoction combined with western medicine in the treatment of DUB can control significantly the bleeding symptoms of patients,ruduce the endometrial thickness,improve estrogen secretion abnormalities,promote the coagulation function return to normal,and the adverse reactions are less,the clinical efficacy is better than the use of western medicine alone.
