摘要
目的:探讨早期应用通腑排毒汤保留灌肠对蝰蛇咬伤患者炎症指标和尿液指标变化的影响。方法:将91例蝰蛇咬伤患者随机分为两组,对照组42例采用常规的方法治疗与护理;观察组49例在对照组的基础上,从治疗的第1 d开始应用通腑排毒汤保留灌肠;通过观察两组患者治疗前后炎症指标和尿液指标的变化以及根据患者的住院天数评价其治疗效果。结果:治疗后与治疗前比较两组患者的血清超敏C-反应蛋白(h-CRP)水平明显下降和尿量明显增多(P<0.05),但观察组治疗后h-CRP水平下降和尿量增多比对照组更明显(P<0.05)。两组患者治疗后的尿蛋白、尿白细胞和尿红细胞阳性率明显低于治疗前(P<0.05和P<0.01),但观察组治疗后这3项指标阳性率更明显低于对照组(P<0.05)。观察组平均住院天数比对照组缩短。结论:早期应用通腑排毒汤对蝰蛇咬伤患者进行保留灌肠,可以起到泻火解毒等功效和起到改善炎症反应和肾功能的作用,缩短治愈时间。
Objective: To probe into the influence of earlyretention-enema with Tongfu Paidu Decoction for the changes ofinflammatory and urinary indexes in viper bite patients. Methods: 91 cases with viper bite patients were randomly divided into two groups.The control group with 42 cases was treated and nursed with routine methods, and the observation group with 49 cases and was treatedusing the retention-enema with the Tongfu Paidu Decoction on the basis of control group, starting from first day of treatment. The treatmenteffects were evaluated according to the alteration of inflammatory indexes and urinary indexes before and after treatment andhospitalization days in the two group of patients. Results: The serum high sensitivity C-reactive protein (hs-CRP) level obviously decreasedand urine volume obviously increased for both group of patients after treatment in contrast to before treatment (P<0.05). But, decrease ofthe h-CRP level and increase of urine volume was more apparent in observation group after treatment than that in the control group(P<0.05). The positive rates of urine protein, white blood cell of urine and red blood cell of urine were obviously decreased for both groupof patients after treatment in contrast to before treatment (P<0.05 and P<0.01). However, the positive rates of 3 indexes were moreapparently decreased in observation group after treatment than those in the control group (P<0.05). The average hospitalization days ofobservation group were shorter than those in the control group. Conclusions: The early usage of retention-enema with Tongfu PaiduDecoction for viper bite patients may improve the efficacy of purging fire detoxification and induce the action of improving theinflammatory response and renal function, as well as shortening the cure time.
